The entire cylinders reside in the tunica. It is possible for the back end of the cylinders to perforate the tunica in the crus and I believe this would require surgery if that is indeed the issue.

It's not important. It just means the tubing connector at the bottom of the implant is parallel to the cylinder so it fits easier in the crus.

It is one of the signs of infection, however one surgeon told their patient that this symptom in isolation is not cause for concern and it can detach after a few months.
