selective ED

Please help
I am 60 Diabetic ok with masturbation but absolutely no erection with wife after several attempts
I don't think pills are any solution but tried few times without much help
Seriously considering implant
Am I right?

With masturbation (by yourself?), how rigid is your erection and how long can you make it last?

If you wife masturbates you (if you can get her to do that or do fellatio) do you get completely rigid (as in enough for penetrative sex) or do you get a semi-hard erection that continues only while stimulation is maintained?

These were my markers for clinical diagnosis of a physical cause of E.D.

Go find a good urologist who specializes in sexual health. I've been a type I, insulin dependent diabetic for 30 plus years. I'm well controlled, but the first place peripheral vascular disease showed up was my penis. Suffered for years. A simple doppler test will diagnose if you are a candidate for an implant. The longer you wait the more size you lose. I should have done it 10 years ago.

Only you can make the final decision whether you are right or not. I can tell you the factors that affected me in my decisions, although they may have different weights for you since I don't have diabetes. In my view, the goal is to maintain as much function as possible with as little medical intervention as possible. For me, this meant starting with pills like Viagra almost 20 years ago when ED first showed up. When the side effects got too bad three years ago, I switched to Trimix injections, which still work well. When the injections stop working, I will look for a good surgeon to do an implant. The fact that you can still get a usable erection through masturbation suggests to me that injections would work for you. Although the idea of injecting into your penis may be daunting, many of us on Franktalk have found that it is not a major issue. It does take a while to find the right dose. Once you have the implant done, there is no going back. However, from the comments here on Franktalk, many men have found that this is the best solution for them. I strongly agree with Fredrickdave that you need to find a urologist who specializes in sexual issues.
71 years old in 2018; married 49 years. ED since 2001. PD5 inhibitors (Viagra, etc.) worked with increasing dosages until side effects got too bad by 2017. Now using Trimix injections. Delayed ejaculation has been a continuing problem since 2012.

My only dilemma is I will get my normal and usual erection with masturbation but none during sex with my wife. This may be a kind of mind issue I agree but solutions like pills or injections are not an option. In such a scenario can I sacrifice my private erections in favor of a penile implant so that I may have sex and satisfy my wife.If surgery go wrong I may loose even my private erections. Though I am a diabetic since I am extremely successful with erections during masturbation that can rule out vascular pathology. I appreciate members thoughts.Thanks

Why aren't injections an option? You can continue your chickin chokin as is and then inject for wife time.
R.R.P 2011 Mayo Jacksonville, Dr. M. Wehle. Not nerve sparing. C in margins. Radiation 2023, V.E.D, Viagra and PGE-1 (80mcg/ml) injections @ 8 - 14 units. Originally Edex20, then compounded PGE due to cost. Inject. 12 yrs. It works. Treasure coast of FL.

ED with wife, girl friend or anyone but can have successful erection when alone in private and able to complete masturbation

Don't understand why injections would not be an option.? Would certainly investigate that before considering implant.
Age 80 good health. Radiation prostate 28 hits 4 years ago with 6 months of horrible ADT. Good recovery and ED worse.
Can't tolerate oral drugs but 5 years of Alprostadil 40mcg/ml 25 units is a miracle for 3 hours. GF is happy!!

I MIGHT be able to answer, "yes", depending on what you mean by "successful erection when alone".

I was unable to have erections sufficient for penetrative sex with girlfriends. But was able to have enough engorgement (a soft, flexible, squishy "pseudo-erection") for masturbation or fellatio sufficient for orgasm/ejaculation. But, at the end, that was all I could get for more than a few seconds at a time, at best.

For years, oral medications were successful and I would recommend using those rather than an implant. If you get five or ten more years of sex with oral medications that increases the chances that a better treatment than an implant of the designs in current use today might be available. Or, perhaps a genuine cure. An implant (as good as it is) is not a cure. In fact, it makes E.D. a permanent 100% impotence because of the damage the operation does to your corpus cavernosum.

What is smallest penile prosthesis that is availble
I am 5 feet 5 inches tall and my best erect penis is at most 5 inches ( 12.5 cm )
