Greetings.

Hi all, been a lurker for 6 months or so...
Age now 67, Raleigh area, NC. PC diagnosed after 3 biopsies in fall of 2016 after several years of watchful waiting. psa was running between 12-16, - Gleason was 3/4. Radical Prostatectomy (DaVinci) 01/10/17 with Dr Becker at Wake Med. Prostate was very big at 104 grams. PSA now undetectable.
Incontinence (SUI), immediately after surgery-12-16fl ozs per day requiring 3+ full Depends. Couldn't stand it any longer so sling surgery on 5/05/19 with Dr Lentz at Duke. Was 100% successful for 10 days then suddenly got worse again, now on 1-2 Depends per day, pads are just not up to the job for me.
Total ED since PC surgery, pills do not work. I am being advised to have implant and AUS simultaneously but I am concerned with having all that mechanical apparatus installed for obvious reasons including reliability and comfort.

Many PCa guys hVe been down your path. I know of several that had both AUS and penile implant done together.

All of them have been very satisfied with the results.

The skills of the surgeon will be the greatest determining factor. Quiz your surgeon thoroughly.

General questions I can think of right away:

Risk of complications when doing simultaneous surgeries is greater than doing them sequentially, but not doubled.

Risk of complications installing one device some time after a prior device installation is also greater than doing just one.

Risk of infection with a single operation to install two devices that takes longer than a single device is greater than risk of infection to install just one. But the second operation adds a second exposure.

Installing two devices at the same time is likely to result in more post-op discomfort than if you install one at a time. If you have really good after-operation care this can be mitigated quite a bit. If you are on your own, not so much.

Cost of doing one at a time involves two hospital charges, operating room charges, anesthesiologist's charges, etc.

Add more targeted questions as you think about the devices and your particular situation.

I was advised against having vasectomy at the same time as my implant because of the risk of complications (if I have anything unusual happening in my scrotum after the operation, there might be uncertainty as to which procedure caused them). But I figured the vasectomy is VERY simple and did not want to have my scrotum opened (even minimally) for a vasectomy later on, so elected to have both done at the same time. I had no problems. (Note, I was 69 years at the time and unlikely to get anyone pregnant, but I still held onto the potential for having sex with a 30-something gal some day. :D )

Very detailed response which is very helpful.. Wow, are you a doctor? Must be in the medical field. Anyway, have you had any issues after your implant?

Well I have heard many patients do get "double surgeries" to fix the penis and the incontinence however you spell it. Don't see any problems having talked to several guys who had the procedure done....They are happy.
