I had that problem all my life and solved it without even intending to do so. Basically what happened is that I had unrelated issues which resulted in me taking Citalopram (SSRI) for a few months. It must have permanently altered my biochemistry because a few months after I stopped taking the drug I noticed I didn't have PE anymore. In fact, I even had difficulties finishing.
I'm glad I didn't take it for too long cause it could've very well lead me to ED problems. But it certainly fixed my PE.
That being said, I would advise caution cause it altered how my organism processes carbs. I basically gained weight, which was difficult to get rid of. Still, with some diet improvements I was able to eventually lose that extra weight.

I work with men clinically with PE almost every day. PLEASE be careful with this therapist. What is their general approach? If they mention the squeeze method, run like hell.
Once again, family doctors don't know anything about sexual medicine...and NEITHER DO UROLOGISTS. ED and sexual issues are never part of a urologists training. A uro is a surgeon. Don't go to them for sexual help. You need to find a men's sexual medicine specialist. Where do you live?

Do you still have the PE issue if your wife makes you cum first, say through masturbation or oral? You can follow up by giving her an orgasm with oral, and by that time you'll probably be hard again and able to penetrate her longer the second go round. Maybe even masturbate yourself before initiating sex, see if you last longer then. I would definitely avoid jelging at all cost, every story I've read on that seems to end with damage to the penis, and ED. I suffered from PE also, but the first wife had it too so it worked out really good. Since having ED issues now, when I take Cialis it takes me a long time to cum, it seems to lessen the sensitivity a lot.
Retired 65 y.o. Married. Moderate ED since 2019. Use constriction band ocassionately to help maintain erection, or Cialis/Viagra. Nocturnal/morning erections returning with VED usage. Lower libido than before.

I do not suffer from pe but I take it if necessary from time to time in very small doses to lengthen the relationship, finding myself well, do you try dapoxetine?

The first line of treatment would be starting with a super low dose of an SSRI like Zoloft. SSRI's are notorious for delayed ejaculation. Some doctors use Prozac....but any SSRI will eventually work. At my office, we usually start a guy at 25mg for two weeks...and every two weeks he increases the dose by 25mg until he finds a good level of control. Most guys end up around 100mg. The standard dose is usually 200mg for mental health treatments.
The physician Finder is a good resource. If you are near NYC, I work at www.mazemenshealth.com. We are a sexual medicine medical center. But this is NOT about promoting where I work. The physician Finder at the top of this page is a good start. But be careful, most doctors are not good at this stuff. A lot of these guys are implant guys. So, shop around.
You should never get an implant due to premature ejaculation.

Used to be lifelong as well and I tried a number of pills , condoms and drugs that usually made me feel like crap however I came across a product called Afromina which has helped substantially. It's derived from an African plant and it essentially numbs your penis. The trick is to find the sweetspot between applying it and still having feeling in your penis vs leaving it on TOO LONG making your penis numb. I found if I leave it on for no longer than 2 minutes provides me the ability to enjoy sex and having control while making her happy as well .I'm sure she ll be happy but you won't feel much. I can usually get 2 sessions with one pkg and it comes with a ( small) condom

Strange but the last two time we have used Trimix, I have been able to go for about as long as I want to. My wife came 3 times and after a good half hour I let loose, nice to have some stamina again. With the confidence of the Trimix I am starting to really take my time.
