I appreciate your keeping the conversation going about delayed ejaculation. It is an under-discussed topic of great concern to some of us. My doc did a QST test (I forget exactly what the acronym is for, but it involved testing sensitivity to heat, cold and vibration).I flunked! The test came up abnormal in most respects. He then sent me for an MRI to check for damage that night be causing the lack of sensitivity. I didn't yet have his interpretation of the MRI, but my reading of it said most functions are normal. I'm hoping - likely in vain - for something that is curable.

I can speak to my experience regarding item 4. I have used TENS type units (electro-stim devices purpose built commercially and homemade) for the last 20 years or so. While I never found them to help with ED (unlike Viagra and trimix) I have found them very effective at inducing orgasms while in use (AKA the HFO or hands free orgasm). In my experience if used for too long at a time (4-5 hours for example) these devices do somewhat desensitize my penis for a day or two. Since I'm the initiating partner 99.999% of the time I always wait a couple days to initiate after using estim as a masturbation aid.
Birth Year 1959, Happily married to wife since 1987. On TRT since age 45, started sildenafil at age 48. Had lifesaving abdominal surgery at 61 with lost erectile function following surgery. Started trimix in January 2023 at age 63.
