I believe this is the category I fit into here.
After a period of extreme emotional stress in August I held on to urine too long and developed a chronic pelvic pain.
My symptoms did come and go but are currently pretty constant.
- tight perrenium that spasms occasionally
- constipated feeling
- sharp pain running the underside of the penis
- nerve tingling sensation all over penis with altered sensations
Not sure if I had pudendal nerve entrapment or just irritation- I suspect the latter seeing aa symptoms are not constant.
Sex can wildly vary. The other. Day I managed it 3 times in an evening... but still doesn't last that long and generally only one position.
Had an MRI scan last week and get results Tuesday.
What do others do for the pain and muscle tightness?

Psychogenic ED since forever fewer problems being fully erect alone.

Chronic pelvic pain pudendal neuralgia and muscle tightness in late 2023 onwards making ED worse.

I suffered from a similar condition for years (constipation, difficulties to urinate, painful ejaculations). I was a cyclist who did at least 200km/w. There is 3 paths you should explore.
1. Is there any condition causing these spasms? I my case, grade 2 internal hemorrhoids. Once I started to get treated, things improved (I guess I felt that something was pushing and my muscles responded in a certain way or all this blood pressed on the pudendal nerve). So go get your bladder, butt and prostate checked
2. Neurological: there is only one way to "see" an inflamated nerve: a perineal electromygram. Forget about the MRI, you won't see a thing. It's a painful exam, though. An other way is to inject an anesthetic close to the nerve and see if it releave the pain. I finally had a an infiltration
3. Physical therapy: there are ways to releave the muscular tension. The best exercise for me was the stretch my pyriform muscle (pudendal nerve pass very close to it.
With this, I managed to live a pretty normal life. I can now remain sit for a longer time. No problem with sport but I accepted that I will never bike again
