Title: Traumatic catheterization, mechanical restriction and urinary problems – urethroplasty or scar release?

Hi everyone,

I’m a 30-year-old male from Hungary. About 18 months ago I had a penile bending injury with a shaft haematoma, followed by a traumatic Tiemann catheterization. The catheter met resistance, was withdrawn and then forcefully advanced.

Immediately after the catheterization, everything changed. I developed extremely rigid, rod-like erections and my penis felt mechanically fixed and difficult to pull away from my abdomen.

I later developed another shaft haematoma and, about four months after the catheterization, a left crus haematoma. After the crus haematoma the rod-like erections disappeared, but severe mechanical problems remained. I subsequently also developed significant prostatitis, which was treated.

My main problems now are difficult urination, bowel movements, erections and ejaculation. During urination it feels as though the urethra cannot expand properly, and I need significant abdominal straining. My Qmax went from about 27 → 20 → 14 ml/s.

During bowel movements I sometimes have to manually push the base of my penis downward against the pubic bone because otherwise the area feels mechanically blocked.

During erections and sex I feel a strong catching/constricting sensation over a short area at the penile root/proximal bulbar region, almost as if the surrounding tissues are being compressed.

My RUG in November 2025 caused five days of bleeding, and cystoscopy in December 2025 did not show an obvious intraluminal abnormality.

A later MRI showed chronic injury/thickening of the left ischiocavernosus muscle, mild bilateral linear fibrosis in the distal corpora, intact suspensory ligaments and a normal-appearing corpus spongiosum.

My question: has anyone experienced something similar where the urethral lumen did not show a clear stricture, but periurethral/perispongiosal scarring, adhesions or tethering caused mechanical symptoms?

If such an external problem were found, would treatment normally be urethroplasty, or could scar release/adhesiolysis be an option if the urethral lumen itself were healthy?

I’m mainly looking for experiences from people who have undergone reconstruction or scar-release surgery.
