Active PD, Severe ED, 25 yo

This is for the guys who experience an onset of curving or bending of their penis. PD is a frustrating condition which often creates significant emotional and mental distress. It overlaps several other sexual function conditions.

Active PD, Severe ED, 25 yo

So as the subject says I will give you all a little summary to get the most of my first post here on this forum.

I have already posted a little about my PD journey on the PD forum so you can all get a good idea of how it all started.

Today I can’t believe I’m here posting because since 2 years this disease has only been taking away from me and just never seems to enter the stable phase despite trying everything other than injections because they aren’t available where I am and tbh no doctor here is ready to do that for some reason and they believe it could give me more scar tissue and pd???

Anyways now I have severe ED, the disease is still active and progressing and I am seriously considering an implant.

I’m young I have a girlfriend who is understanding and I just think maybe the implant will make me straight again and give me reliable erections for some time and PD can be long gone.

Should I do that considering so many people here report of returning curvature post implant?

I mean I really don’t want to waste resources if the fact of the matter is that i am just fucked and have been deprived by this aspect of my life for whatever reason and genuinely call it quits on certain things, leave my girl, live a life of celibacy or isolation cuz I mean better than getting surgeries every few months cuz PD is just absolutely indestructible

Welcome brother, Your situation is not hopeless. I'm sure there will be a solution for you. And having a supportive girlfriend is a wonderful advantage.
After sudden onset profound ED from prostate surgery I used Trimix injections for a period. That built up scar tissue as a result, even though I wasn't using them for all that long. When I eventually had my implant procedure my surgeon told me he had found scar tissue and removed some of it. I think often they have to leave some behind so as not to compromise the structural integrity of the penis.

Once I started cycling my implant I noticed that I was slowly developing a curve. It got to a certain point then didn't get any worse. Surgeon diagnosed it as PD. He has me doing twice daily bending exercises to hopefully resolve it over time. I think the medical term for that therapy is "shaping" or "modelling". He told me it's a long game and not to expect improvement for 2-3 years. I have noticed very small improvement after about 6 months. Regardless of that I haven't found the curve to be a problem at all. I regained my original penis size and I have a rock hard erection on demand any time I want one. PIV sex is fantastic. The misery of ED is in the past and I orgasm 100% of the time. I've also found the curve to be an unexpected advantage, as it mimics extra girth. Sex now feels much tighter inside a vagina.

I have an AMS LGX, which is less rigid than some other inflatable implant types. I'm happy with it but, with the wisdom of hindsight, I think both my surgeon and I would have gone with the CX model. It's considered more rigid and could potentially help with a Peyronie's curve. The Coloplast Titan is seen to be possibly the most rigid IPP available. There is an improved model due for release soon, which may have better longevity. You are a young guy and current implants have a limited lifespan. So you would be looking a potentially several revision surgeries over your lifetime with current technology. But that is always evolving. You could also consider a malleable implant. They are much simpler and can last much longer but both types have their pros and cons.

I suggest doing as much research a you can on here, ask questions to get an idea of the options that might help.
I'm also happy to answer by PM. There are guys on here with similar backgrounds who have gone down the implant path and are very happy even if they still have a residual bend. Then see the best high volume implant surgeon/s you can find, armed with that information, and ask what they recommend.

I’m truly grateful for your reply. I understand that residual curvature can be left there and that’s fine too honestly.

What’s very unclear in my situation is the fact that I have diagnosed PD which is still active. I keep losing size which is impossible to bring back even with implant. Okay even if I make peace with the fact that I have now lost more size with or without an implant a 5 inches is not enough to satisfy any woman in my opinion.

I read a lot on the forum that guys with ED who ended up going for an implant like yourself found post op that there was some form of PD present which was probably so mild that it went unnoticed throughout your journey hence easier to deal with in surgery too. Secondly it must be stable unlike mine. Mine is still progressive, if I use a little logic a wound healing disorder that is still aggressively damaging my penis, how would it react when it’s cut open for implant. At first I thought okay implant is my last resort as it will give me a straight strong erection despite however my fibrosis or plaque is behaving but after knowing what I know from all of you guys it will even bend the implant??? And then with this disease I’m forced to think that every person is somewhat different and I have throughout been very very unlucky with the disease so my own personal trend is also very very messed up already.

I genuinely don’t want to sound hopeless, and I’m not writing here just because I want to share my experience i genuinely want to know what I should do. I am from Pakistan so there aren’t a lot of specialists here. Most implants are being done in the USA, I’m currently trying to fly either to UAE or USA whatever happens first and go see a specialist there. That’s the most I can do from a humanly aspect of seeking treatment but then there’s the actual treatment, even with implant if I already know that it’s not gonna be a fix why do all that?

I did consider a malleable but they almost never fix any curve and don’t provide any girth gains. Though the longevity is very very attractive. I don’t understand anymore what to do.

I have read so much about all of this stuff, yet there’s only one thing that actually makes sense which is that I’m actually fucked and might need to close this chapter, he celibate, and live like I have a condition cuz I can’t be a normal man either. And TBH from what you have just told it makes sense but then again my PD is unfortunately still active at 2+ years so idk if anyone would perform surgery let alone the fact that I’m 25 only and cuz of that doctors don’t even think I need an implant. The last doc I went to said if anything you’re recovered and then told me if I’m not happy I should fly to UAE

There is this other doctor, he said he can do the implant on me but without giving me a clinical erection, without doing a photographic analysis, without accounting for severe worsening ED he claims that I’m stable. So it makes me question does he know the PD is gonna come back post op and most probably way stronger than before cuz now there are actual cuts and bruises that need to be healed and my body has already showed it can’t do that without developing fibrosis

Despite having some well endowed guys on here, the world average erect penis length is something like 5.3 inches. I believe studies have shown that 5 inches is more than enough to satisfy most women. But I do understand your concern about losing further size. Using a vacuum penis pump (VED) daily can help minimize or even reverse loss. I did that when I was losing size due to penile atrophy caused by the ED from my prostate surgery. I was able to turn things around with VED therapy and eventually restore my original size. I don't know how that would interact with active/progressive PD but, if your doctor approves it, I would start immediately. A combination of that and a properly measured fitted implant can restore size over time, with healing and diligent cycling.

But I can't say how your body would respond to the healing process. As mentioned I also had some fibrosis, but it didn't affect my implant surgery recovery other than what I already outlined. It would be good if some guys who have gone through your same condition reply to your post, especially if they chose to be implanted. But I think you really need an expert medical opinion on that in relation to active Peyronie's.

If you can afford to travel to the USA for surgery I think that is likely to be the gold standard for any treatment that can help you.
But you could also set up a virtual consultation before taking things further. There are surgeons in the US who will have seen cases like yours and have been able to help. Drs Hakky, Eid, Clavell come to mind but there are many others.

It might be helpful if you also cross post in the Implants forum. It gets a lot more traffic and may get the attention of some PD guys who have been implanted.

My original message remains, please don't give up.

Yeah I guess you do kind of get me and my situation now. Even if I am to fly abroad and get surgery it’s gonna take months and considering how aggressively I’m progressing maybe I am chasing something that’s not meant for me anymore.

Thank you for your lovely advice and hopeful outlook. Wish you all the best
