Xiaflex Injection Protocol

This is for the guys who experience an onset of curving or bending of their penis. PD is a frustrating condition which often creates significant emotional and mental distress. It overlaps several other sexual function conditions.

Xiaflex Injection Protocol

I am trying to find a Urologist who specializes in Peyronie’s Disease and who is successful with Xiaflex. The gold standard of treatment seems to be Dr. Landon Trost, but he is not in my area. Prior to injecting Xiaflex into the flaccid penis, Dr. Trost injects the penis with a medicine to induce an erection. He then measures the length, girth, and curvature of the erect penis and examines the penile plaque. He also uses an ultrasound machine to measure the location, thickness, and density of the plaque that is causing the curvature. The other urologists that I have consulted with, who also treat Peyronie’s disease, do not perform these steps. Are these steps necessary? I am curious what others experiences have been.

My urologist did a manual examination prior to each round of injections. The procedure went like this:
Injection one was a nerve block in the pubic area.
Injection two was to create an erection.
Next step he palpates the area and marked the limits of the plaque where the xiaflex is to be injected.
Injection three brought the penis to a flacid state.
Injection 4 was a local block on the penis.
Lastly, the xiaflex was injected, usually multiple times in varies areas within the margins he marked earlier.

I went through a total of 6 cycles (2 injections spaced a day apart) followed by the recovery and modeling period. The injections along with daily oral meds (5mg cialis and pentoxifylline) and consistent traction brought my from 47 degree down to 25.
56, married 33 years to an amazing woman, BPH, PD (47deg down to ~25deg), current Xiaflex treatment with Pentoxifylline & Cialis. Penile modeling with Restore X. Occasional ED triggered from pain response during intercourse.

Thank you for sharing your experience and I’m really glad that things worked out for you. It sounds as though your urologist is experienced with Xiaflex. Neither one of the urologists who I spoke with, inject the penis to induce an erection like yours did. Xiaflex is such an ordeal to go through that I want to make sure that I get it right the first time. I’m 66 years old and like you, I’ve been happily married for 32 years. Dealing with this horrible disease is made more bearable with the support of a loving wife. I couldn’t imagine dealing with this as a young single man in my 20’s. Thanks again for your input. I really appreciate you for sharing.
