I hope you all had great holidays. I hope I don't get annoyed with my questions. I hope someone will read this and give me their opinion. I want to give more information about what has happened to me and it is still happening.
March I had a penile fracture with bleeding from urethra.A few hours later, I had surgery.
30-40 days after that I had a very good erection. I was happy and everything was wonderful. This lasted about 3-4 months.
4 months later, the erection began to disappear during sex and I lost some of the sensation in my penis during sex.
It happened suddenly. The erection just disappeared and I couldn't finish the sex.
I became extremely worried. I started going to urologists. I had hard-flaccid.
Several times after that I could get an erection without it falling apart.
Suddenly I got premature ejaculation.
I drank Epimedium several times and the sex was like 18, 2-3 times in one night.
But I was still worried and read a lot. I started to have nightmares related to my condition.
My morning erection was already weak.
I was taking Tadalafil 5mg daily for 20 days and 20 mg tadalafil on demand - it had no effect , but my testosterone is Extremely low levels of total and free testosterone, below the minimum limit -
7.01nmol (8.64-29) total
0.173nmol (0.236-0.680) free
I had no desire for sex. None....
Then my erection changes started - sometimes it started normal and disappeared within minutes.
I started to cum with an erection of about 40%.
Just before the ejaculation, he softened. Then it changed.
I started to have an erection (85-95%), which lasted longer (I could hold it for over 10 minutes, BUT ONLY WITH GIRL, I couldn't even reach 50% with masturbation),
but when it penetrated the girl fell off in 2-3 minutes. Then I could take it out and the erection again was 95% in seconds.
Already ejaculation becomes very firm and achieves almost 100% erection. I have a doubt that the ligament is also damaged because the erection hangs slightly to the left, even as it is 95%.

I forgot to add ... Whenever I get an erection the head of the penis fills up (if that's important at all)
Both cavernous bodies fill evenly.
And when I have sex the penis is twisted, From above I can see the spongy body because it is twisted to the left.
When the penetration is a little more difficult (not very difficult, but only a little) I feel something like a leap, tilts to the left of the base.
Like skipping a knee when there is no stability.

sounds like you have more than one problem. Your low testosterone needs to be addressed as well as the physical damage to your penis.

I’m not a doctor and I don’t think too many other members are either.
The last thing anyone wants to do is give bad advice that could create even more problems.
Based upon what you’re describing, you have a lot of different medical issues going on concurrently and many of them could be a problem.
It’s like when your wife buys a bunch of pretty new patio furniture and you take the boxes home thinking assemble will be a picnic but once you open them, nothing looks like the picture, you discover you are basically building the furniture from scratch, and they didn’t include all the hardware you need. When all else fails, read the instructions.
When it comes to bizarre penile behavior after injury involving medications, when all else fails, consult a doctor.
Find yourself the best urologist you can, hopefully one who specializes in the treatment of erection problems. Tell the doc the truth, the whole truth, and nothing but the truth.
It’s not the same deal as mine but sometimes doctors don’t listen. I had seen close to a dozen doctors who dismissed my ED as all in my head until I took my wife with me. One doctor finally asked my wife what she was observing. And once she said I got hard as a rock but once inside her I seemed to get longer, softer, and flatter(less big around) they immediately booked me for the series of tests where they found bad venous leakage. This was back in the 80s and even then they weren’t sure that venous ligation and a crural tuck (which was just beginning to gain favor, and has mostly since been abandoned ) would fix it. Thank God shots helped even if they delayed ehaculation.
You need a doctor who will listen to the entire situation, then order appropriate tests. Even though I like to think I’m real smart I wouldn’t begin to guess what all you have going on.

Sounds like the surgery may be causing the twisting issue. I would also strongly suggest using a water based lube with sex, helps with penetration.

You really do need a thorough medical examination by a qualified urologist who specializes in treating ED.
I suspect you have several problems but none that can't be solved.
A few things stand out:
1. Low free testosterone - You have to get that fixed. Your sexual activities will probably never get right if you don't. It's essential.
2. Fading erections - This sounds like venous leakage, and is fairly normal as we age. As my doctor told me, the valves in our cardiovascular system are tissue, not metal. Over many years, they fray and start to leak. This, too, can be compensated for by known, effective therapies.
For all of your issues, you need to see good Urologist who specializes in treating ED.
It sounds like you have a good, stand-by-her-man wife. I hope so. So many men do not. Her support for your treatments will mean a lot to you.

Health is very good. ED began from venous leakage (2003).
Used Cialis (2003-2017), penile injections (2018-2019), 4 implants (2020-2021), now have an AMS 700 installed.

Hello again from me. I went through a Doppler test a few days ago. I was given Alprostadil and managed to get an erection of about 30-40%, which disappeared quickly. The fact was that I was extremely stressed and worried (although the doctor was great). When I got about 30% erection, the doctor went into the office and the study began. Then he left me again 2-3 times and finally told me that I had no problems with either arteries or veins. My question to you is -
how can it be understood that I have no problem with venous drainage after not getting an erection.
I saw a diastolic value of +12.0 (WOW) on the screen then it started to fall again to +2.0 and I didn't understand anything. At one time I saw -3.00, but briefly, then again some other values ......
Is there anyone who knows what this means
