Fascinating info, as always.

So, in your opinion, how can we counteract these two unfortunate effects of aging: loss of smooth muscle and increase in adrenergic inhibitory tone (I suppose alpha blockers may be useful in the latter case)?

Limpbiscuit just mentioned the P-Shot or Platelet Rich Plasma injection, could this be it?

First steps would most likely be; a healthy diet low in sugars and fats and restricting carbohydrates. Diabetes is an almost sure path to those conditions.
Regular exercise, with an combination of strength training and cardiovascular work. This is without doubt second in line with diet in importance.
Low alcohol intake.
Trying to combat stress levels. Excessive stress puts the body in fight and flight mode constantly. This increases oxidative stress.
Trying to combat oxidative stress with antioxidants, such as Pycnogenol. Oxidative stress is one of the main players in the above conditions.
Encouraging hormone levels to be optimal. Diet and exercise help here considerably.
I shouldn’t have to mention smoking these days. If any of you are smokers, well just forget trying to fix anything else. Your doomed.

As I mentioned, PDE5 inhibitors have been shown to combat the cascade that excessive oxidative stress causes as we age.
Alpha blockers may help with regard to the increase in adrenergic sensitivity.

The P-shot may help. However, this is what I consider a quick fix treatment and often only has temporary results. Something we try when the damage has already been done and is most likely too late for many who try these things. For those who have already made significant changes to their lifestyle and caught problems early enough, these treatments may assist the healing process.

Unfortunately individual genetics can make it an uphill battle for some us. But all we can do is try!
Age 57, ED issues for 15-20 years. Testosterone replacement with Enanthate and Ovidrel. Currently using generic Tadalafil 2.5mgs and Resveratrol daily.

Pycnogenol is one of the ingredients of PRELOX, an excellent food supplement for ED.

Its patented formula includes pycnogenol and l-arginine, both substances seem to work sinergistically.

The only reason why I quit PRELOX was because it seems to increase T levels and was worsening my hair loss.
