I have heard that ED can cause penis shrinkage over time. Now my question is if one has strong and lasting nocturnal erections in spite of venous leakage, does it prevent shrinkage for sure?

A radical prostatectomy will do the same if one doesn't "stretch" it out via vacuum pump or injections. Of course an penile implant will do the same if one uses it.

If the erections are staying the same size over the years, that is pretty strong evidence that you are not getting shrinkage.
