I don't get it
the video mention meta analysis which claims embolization surgery having a overall combined clinical success rate of 60%.
That's pretty high, considering the clinical studies contains outdated surgical techniques from the 80's.
Then the video goes to show how improvement in modern day surgical techniques (sclerotheraphy) have allegedly raised the success rate to 88% 1 year post op with no complications!
How come there are so few actual success stories out there then? Do all the satisfied patients just go back to living normally while the botched cases ends up on forums like frank talk?
The guy making the video, Issam kably (board certified radiologist) says he's open for Q&A at theIRtist@gmail.com. Suggestions on what we should ask him?

It depends on the aetiology of the „venous leak“
I have had the classic dorsal vein ligation three years ago and it helped significantly.

Do you remember what your edv was measured to before the ligation surgery?
Any relapse since?
Can you stay hard (with pills) through the whole act?

The video is quite interesting if you look at his other videos there is a part one that I watched as well. Part one just went over erectile dysfunction and explained interventional radiologists role today. I’ve seen two urologist and both of them had very similar course of treatment for venous leak. Basically it’s oral medications until failure to respond, then injections until failure to respond, then implant. In frustration I’m always looking for new treatments that are coming out in hopes that some day soon a better solution might become available. Actually my current insurance doesn’t cover implant surgery so I’m kinda screwed. I’m an average working class person but I’m still fairly young. I have no idea how in the hell I will afford the implant surgery when the time comes. I guess once the injections start to fail I will work my dick in the ground and see how much over time I can bank to save up for the implant. For now I’m on medium dose injections which seem to work. Let’s pray they last for 10 years.
By chance I came across this video and I will certainly sent him an email. Neither of my urologists even mentioned a CT scan to show exact cause of venous leak. All I had done was a venous Doppler. The Doppler is nothing more than an ultra sound and a very vague way of concluding venous leak. Heck my first Doppler showed no leak because I was injected with enough medication to give 10 elephants and erection for days by accident. I believe the nurse calculated wrong. Needless to say I almost had to beg for a second Doppler which should of been free because the first one was done in error. My second Doppler showed a venous leak.

He did not specifically write the edv down beforehand, I think it was over 7 cm/s.
PSV was >30
In the surgery they discovered two ectopic veins and sealed them off as well.
Especially the glans hardness was really good after the surgery, thats what went back the most.
With daily tadalafil (20mg) I can get and maintain erections most of the time, if I want to be certain I add vardenafil (40mg).

I emailed the doctor from this post (Dr.Kably). I basically told him about my venous leak and asked if he had any suggestions. He responded with a link to schedule a video visit. I was hopeful more people on here would of responded to my post. I might do a video consult with this doctor just to see what he says.

Seems we share a lot in the way ED affects us. I am little younger (born 97) but I already saw you suffered veinous leak and tried shockwave therapy. Recently you had said not only did it not improve your condition but it had had a significant negative impact. I understood from your different posts that you had vein litigation that quite worked out and possibly shock wave reopened them. Since you didn’t update that subject, could you tell me now with little more time if the results remained disappointing or if you start to reap benefits of such an operation and it didn’t affect your veins litigation.
So I address you here in order to get maybe one last try before going to implant. Sorry if that’s not exactly the topic here, I could have easily sent you private message but I think and I read before point of this forum is to offer the biggest chunk of data to us all so that one can benefit if reading it by finding he experiences same ED as others and what they’ve been through.
That way I judge that I may be better to share here on this topic. I promise it’s entirely related.
Just a little about myself. Got ED all my active sex life. Can remember of crazy lasting erections in the past but i have to go back to when I was 14 or so. Despite my problems I have quite managed last 5 years to have sex thanks to pills. I take very high doses but let’s say until recently it has really helped. Pills didn’t make a pornstar out of me but I could have sex easily without too much worry with tadalafil 20mg before sex. More recently I started to need to take 20mg twice a day to be sure. Eventually did 2 sessions of shockwave with Medispec but It only brought pelvic pain. So I stopped before reaching the 6 recommended ones.
What is funny is that until yesterday I never really knew what caused my ED. Uro had all told me it was psychogenic and they had almost convinced me. Not that my dick worked perfectly well, but that the cause was credibly mixed and if I could solve the psychological part it would work at least for few years ahead. Anyway, following shockwave sessions I got a bit stressed and asked for the first time my uro to let me try injections. He tried in his office 5 microg PGE1 but almost no response. Told me the dose was maybe too low and gave me prescription for EDEX 20. And then something happened that literally changed my view on my ED. I injected myself 13 micro of EDEX today, not the 20 because I still feared (funnily) potential priapism. And I noticed something that for sure I could have noticed earlier, but that was so obvious with injection. If I lie down I get an erection strong and sustainable as I have never experienced in years. But if I just try to stand or stop lying, my whole dick fall aparts. I had noticed prior to that I had hard times maintaining an erection standing but thought it was I felt more pressure that way. The good point with injection is that it’s not supposed to be influenced by your mood, so this time I understood the root cause of my ED was definitely organic. In a way I was relieved, it makes my choice easier right now. I don’t need to wait, to go in 3 months of sexotherapy... I have to find a way to deal with my crazy venous leak, honestly it’s impressive I think. My dick goes from full erect to almost flaccid in 15-30sec when not excited and under injection.
Sorry for this long story. My point is to talk about venous leak solutions. I know now that I know the cause is organic I am approaching implant decision, last resort for us all. Though I thought maybe let’s just try this surgery before. Anyway from what I read I don’t have so much to lose. My question is there : what is your experience in vein surgery ? It seems it improved your condition but in what extent ? Did you respond to pills before or not ?
Plus there is just this question in my head that seems to be stupid because no one seems to talk about it. I understand the surgery is done with cavernograohy and scanners. Isn’t it supposed to affect fertility ? Or am i completely wrong
