Intermittent erections
55yo, fit, healthy man with 20+ year history of ED. I'm finding anymore that I simply CAN' NOT get a good quality erection for penetrative sex despite higher dose Viagra, Cialis, Trimix, and use of a VED. As much as I hate to admit it, in viewing amateur porn (alone) though, I can usually get 2-3 very hard erections. I'm sure I'm not the only man in a monogamous relationship that suffers from this type of performance failure. My wife and I have not watched adult videos together before and I am very hesitant to bring it up and imagine her thinking' "does he have to look at other naked women to get it up? Would you push forward with implantation with this type of ED? I am becoming desperate and saddened that I haven't had intense, quality sex in a very long time.

Sound like a lot going on here to me. My first thoughts are that you are not as turned on by your wife anymore (Psychological issue here) and you have a venous leak (Physical problem) because you can get hard 2-3 times while watching porn and lose it. Also, you say TriMix is not very effective.

You may be told this sounds like porn induced erectile dysfunction or you're no longer attracted to your wife but I don't necessarily think that's the case. I'm incredibly attracted to my girlfriend and we have amazing sex, especially when I'm on Trimix, but I've also noticed that on non Trimix days, when I'm only using PDE5 inhibitors, often high dose, I'll have a performance failure with her but then be able to masturbate two to three times to porn after we've parted ways. I think this is more a matter of my performance anxiety when we're together. I know it sounds ridiculous that you can have performance anxiety with someone you've been with 100+ times but that's what happens with ED.
Also, as 68CatFan mentioned, I too suspect I have a venous leak (have a penile doppler ultrasound set for February to confirm) and my erections are up and down. When I masturbate, I can work through this and regain my erection if I go soft or lose firmness throughout the session. When I'm with a partner, say I begin to feel myself losing firmness when I'm inside her, I get stuck in my head and we end up mutually masturbating but even then I'm not as hard as I am when I masturbate alone and that's not on her. That's on me for being full of anxiety and adrenaline because I'm in my head so much.
I'm riding out the injections for as long as I can until they're no longer effective or there's fibrosis. Daily lower dose Tadalafil. Higher dose of Sildenafil on non injection days. I'm somehow having the best sex of my life at 48 years old despite my ED. But I've already discussed the implant with my girlfriend and told her it's something I'm going to do once the injections stop working in addition to the PDE5 inhibitors. Are you and your wife able to talk about your ED at all? Does she know you've tried the pills, injections, etc. or do you fear that too would make her feel as if she's not enough to turn you on?
As far as introducing porn into your relationship, approach it in a manner as the two of you learning what turns the other on. Exploring fetishes and kinks. Tell her you'd like to watch a movie and just touch or go down on each other during it. Ask her what she's into or what she fantasizes about and find content you think she'd be into that also turns you on. I found out that my sweet girl next door girlfriend is super into kink.com productions and we've been able to incorporate blindfolds, bondage and other kinks/fetishes observed in those videos into our sex lives all because we watched porn together and play on what we've noticed turns the other on.

48 years old, ED since teens, 25-50mg of Sildenafil worked for 20 years but now require 100-200mg of Sildenafil - sometimes split into 25-50mg every 4 hours - + daily Tadalafil at 5-20mg. Have had P Shot, Gainswave, TRT, and now on to Trimix injections.

In my young single days I had no ED whatsoever with women of my body type: thin. But I had minor ED problems with chubby girls, and complete ED once with a hefty woman. After figuring out that my subconscious was a problem, I only dated thin women. No ED for decades, until I starting getting old.
As Freud told us long ago, the subconscious can hold unfortunate stuff. What to do about it if it's not body type? Somebody smarter than me chime in.

Age 79 in 2024. On testosterone replacement due to hypothalamus malfunction. (Attention depressed guys: low testosterone is a cause.) Healthy health nut but ED due to getting old. Like to keep enough cardiovascular ability to thrust for 30 min.

tobenormalagain, I often see posts like yours and I immediately wonder have you really turned over every rock before going to implant? I spent a great deal of time starting 3 mos ago to turn over every rock. Nothing is a panacea, but I think PT-141 (bremelanotide) and supplements like L-Arginine don't get enough inclusion in the standard bag of tricks that people should try before resorting to the draconian implant.
ED since mid '20, with slight Peyronies. Thus begins a search for the right solution(s); 56yo a/o Sep '21. Three months into my quest (12/21), I think PT-141, Cialis/tadalafil 20mg and L-Arginine (maybe other herbs) are a winning formula. For now.

PT-141? I've never heard of it and where is it even obtained? Regarding turning over every stone? Admittedly, I've not tried every traditional and off label treatment, but most. To be honest, I have expended so much mental energy trying to understand the why and how of my ED and sadly, ED was a contributing factor in a prior divorce. Now that I am remarried, I want a permanent solution and not the constant worry and stress of performance anxiety. Basically, I would love to f*ck all night and have incredible, passionate sex with a special lady!

I think my wife would be horrified about watching porn, but we are 80. The generation of today have no problem at all watching porn, solo and together while masterbating. We were just born to soon. Maybe a sex therapist would help.

Search here on PT-141, some useful discussions on it. PT-141 is a gamechanger for me when coupled w/ a PDE5 inhibitor like Cialis. It's not something your doctor can or will prescribe (or maybe know much about depending how much they have their head in the sand) but it's readily acquired from US distributors selling research peptides. It will get you to the f*ck all night state if that's what you want (you must be a marathon runner).

Similarly for me. Failed treatment with an alternative therapy against prostatitis (transutheral cold laser, which was supposed to deactivate the inflammation foci - fraudulent BULLSHIT, as I know today). After that erections much worse than before; almost no more spontanous erections as I used to have before that "treatment". My initial suspicion was that the nerves around the prostate were damaged (still believe this). Symptoms: penis and scrotum rather cold, blood circulation in the penis does not work really well. What I did against: Since then I take 5mg Cialis daily. Result: Massive erections at night (so that he sometimes even hurts in the morning), in normal sexual situations (wife) though hard to get an erection, and, to be honest, in extramarital situations it's a lot easier because of the probably higher dopamine level (the pruritans may forgive me for that and don't judge my charaker hastily). All in all: For sex I boost my daily cialis with 50mg Viagra. Strange: When watching porn I can get a 100% erection and can hold it for well over an hour.

Strange: Right after the laser treatment my congenital curve went gradually (within one year) from 25 to 35 degrees (there it stopped thanks god), but no Peyronies at all, says doc... Shock wave treatment was without any success. I have now the last hope that arteries habe been tightened by that treatment, which could possibly be corrected with a stent treatment.
My main problem is that it is much too bad for carefree and pleasurable sex like it used to be, but it is far too good for an implant, and after this condition has been preserved for many years now, I think it will probably stay that way. But what could be the cause, what is exacty wrong in my erection system? No urologist (and I have visited the very best) has ever found out.
