Seeking Advice on type of Provider to see

First post, if this isn’t the right place for it, I’ll move it. Minor ED started 10 yrs ago, managed by Levitra and later Cialis. 2 yrs ago, I got Covid (minor case) and things started to rapidly decline. Around the same time, I also started T pellets at the “recommendation” of a friend. Between the two, or completely unrelated, things progressed quickly to the point that max dose of pills wouldn’t cause much of a reaction despite stimulation with my wife.

After a year and a half of playing the game “will it work today or won’t it” along with pills, T pellets, GainsWave, P shot, pumps, counseling (because it’s in your head), acupuncture, ancient Chinese herbal tea (true story), potions and supplements… I went to a urologist at Mayo to make sure there wasn’t anything physically wrong. I did not seek anyone in particular, but live in S MN and happened to see a provider who is well known on this site. After receiving tri mix and an ultra sound, it was determined that everything was in working order. Except, it still wasn’t working.

I was prescribed trimix and starting using it (very successfully!), but had 2 instances of priapism in first 5 months that resulted in spending Saturday nights in the ER. But, IT WORKED!!! I used 15 units initially and went to 8 units after the first ER visit. Then, had a second incident and have cut down from there. The problem is that 8 some days doesn’t result in success, yet on other days… the same dose puts me in the ER even after trying all the tips and tricks to get it to go down. I have read all the posts about priapism, how to manage, is it a concern, etc… I finally have another appt with this (very busy…) Urologist in early July to discuss other options. I realize I am not unique, but wanted to provide background as I seek advice. So, here it goes:
Is there another type of provider I should seek out to ensure I have explored every option available short of implant? I’ve already tried a Men’s Health Clinic w/ NPs, counselor and a Urologist.
I am not afraid of an implant, I have an incredibly supportive wife, but I will have a lot of questions should it come to that. I know it’s irreversible, so I want to make sure I have exhausted all the options. Thank you in advance for your advice and this wonderful resource!

Sorry you were having these troubles! They are not fun… It sounds like you were very resourceful with going to Mayo… And found a very reputable doctor or at least one that has been mentioned frequently… Perhaps a suggestion would be to search online for a urologist that specializes in ED and not necessarily implants for now… Even though the one at Mayo was mentioned many times and all of that perhaps just a second opinion outside of his opinion and course of treatment. But I don’t think anyone other than a urologist would be able to help this area…Good luck!

Dang your story is like so many others here. Sorry. ED stinks.
You want to see a men's sexual health urologist. That will be the one who can do any tests to determine why you're having issues. They can offer options, but it already sounds like you are headed to bionic land. When you find a doc you like don't hesitate to ask questions. Don't be shy because they have heard it all many times.
I started with pills, those worked for a few years then the side effects got worse besides not getting me an erection. Injections worked great for about 3-4 years but always increasing the dose to make it work. When my dose got really high it would make me ache all the next day. Then even injections began to fail.
The hassle with either was timing. Do I take a pill tonight? Got warmed up but now I have to jump up to go inject? Both forced us to schedule sex, a real mood killer.

I have struggled with ED for 10+ years, and recently started Trimix. I am not a medical professional, but a user like you.

Your story is quite intriguing. 8 units is fairly low dose. If some days it doesnt work, maybe you missed the target?
I've heard here (on FT) that for some guys the injection works differently depending on which side they inject left or right side.

As suggested by another user I started little spreadsheet, with the date, left or right side injection, how much I injected and how long the erection lasted.

I would suggest asking your urologist about the Trimix strength, because when I first talked to my urologist, he suggested starting with 10 units, which for me seems to get about the same results as viagra. Which seems I can get fairly consistent results.

Now if I can only get the wife on the same page... (my second injection a couple months got a zero. best I can figure I missed the target. she would talk with me for two days, as it reminded her of viagra not working.)

PS To answer your question on the type of provider, I would consult with another urologist about getting more consistent injection results, which as you have said "very successfully"

Sogwap, that's a good point about keeping a spreadsheet to see which injection side had what reaction. I didn't think of that when I first started to inject. I suppose because it was enough of a mood killer that i was injecting in the first place. Thanks for the spreadhseet tip.
