I've noticed weaker erections for about one year. I've tried daily tadalafil which does produce more "erections" but they are still far from full and disappear within a minute, especially if I get up or roll over in bed. If I lay perfectly still I'm able to get about a 75% erection. I don't watch porn and haven't for over two years, so please spare me that lecture. I also want to note that the daily tadalafil causes some eye irritation for me. My eyes are often slightly red and itchy. This started just two days after starting.
Sex with my GF was great but I've battled PE for much of the last year when I was able to get hard enough for sex. Basically upon entry I came. Luckily, I'm good with my hands and I'm still able to be intimate with my GF but it's not the same. She performs oral on me and unfortunately that's about all I can do. I orgasm with about a 70% erection.
I recently decided to get checked out by a local urologist. The first appointment resulted in him not listening to a word I said, telling me it was either in my head or due to relationship stress (I've had a girlfriend for about a year and a half and I strongly disagree. We had a great relationship and still do, but as you all can relate I'm sure, this issue isn't easy). At my request, he begrudgingly agreed to do a doppler.
The results are below (note that I did not achieve an erection):

A few days ago, my doctor called me with a completely different tone. He was empathetic and told me with numbers like that, I had one option: injections. He told me I'm too young for an implant.
I'm extremely fortunate in my life. I have a great family, friends, a great job with a boss I love, and a girlfriend who loves me. With all that in mind, I can definitively say this is the toughest thing I have ever gone through. I'm just looking for advice.
I'm considering an implant because I just don't think I could inject myself regularly. I hate (HATE!) the fact that I'd be opening myself up to surgeries basically once a decade. I don't worry that much about infection because the odds are so low, and I would definitely go to a high-volume surgeon like Eid, Clavell, or Perito. Luckily, money isn't an issue for me and I have good insurance. But I fully understand there are risks.
I'd love to hear from young guys who bit the bullet and implanted. Alternatively, I'd love to hear suggestions on next steps. I'm thinking about getting my heart and hormones thoroughly checked.

I play tennis two more times per week and in the summers I also play softball twice a week. I get at least seven hours per sleep at night and don't have diabetes. I'm thinking I'm just unlucky.

I'm not a young guy but your story is pretty much the same most of us guys have/had. No one knows how ED begins, why it happens or at what age. All I know is it's not good.
Fortunately, you have a supportive gf, enough cash to go whatever route you choose, and finally a doctor that listened.
Injections are more than likely going to be your first trial. If you didn't get a stiff erection during the doppler my guess is injections are going to be a maybe they'll work, maybe they won't, or maybe they'll work for a while. There are devices available to do the injecting for you if you have to go that route. Truthfully, I hated injections when I was on Trimix. I used an auto injector, but it was still a hassle.
I'm sure if you scan to the Young Members section here you should find more answers from guys your age. There are many 20-30 something guys who have gone the implant route.

Appreciate the response! How old were you when you received your implant? Would you be hesitant to implant as a 30 year old given your experience?

I'm not implanted yet but I likely will be soon. Your story is similar to mine. I have no comorbidities at all. I'll probably post my experience once I'm implanted.

Believe me, when the pills stopped working for me 7 years ago, my friend and pharmacist told me about injections. I went without sex for 7 years because I just couldn't get my head around injecting. Last thing I was going to do was stick a needle in my penis.
Fast forward 7 years, I start on Ozempic which entailed sticking a needle in my belly fat. Then got onto Peptides PT-141 and Melanotan 2, more needles in the belly fat.
So I started to lose my fear of needles and I wanted to start having sex again especially after losing so much weight.
Now I don't even use my autoinject device. I actually love to plump up my dick in the VED and then gently find the spot and then bang bang for a long time. Even my PE is gone, with the confidence in not losing my erection has led to control (plus some edging work from my massage gun).
I would personally try Trimix and see how it goes. You have nothing to lose. If I ever have an implant however I would go for a malleable from all I have read on this forum. Plus I don't mind a bulge in my pants at my age. LOL

ED began around 55 years old, implanted at 68. Not much info available about ED until Viagra became the miracle sex drug. If I was 30 and knew everything I know now and ED started, pretty sure it would be straight to the surgeon's office. But I've taken that leap and know what the other side is like. Like most I did the pills then injections until both failed before learning about implants. I went into surgery knowing nothing except it would allow me to have erections again. Found Frank Talk 5 months post to learn everything I should have known prior.

I’m 23 bro, get it if you want but be prepared for psychological trauma it’s not an easy surgery.

Why'd you get a malleable at such a young age? Overall, are you "happy" like your signature says or no?
