I'm 34, Irish. Today I finally had a consultation with Dr Paul Hegarty in Cork. Ireland. He's the only implant surgeon in Ireland that does young patients. Does 30 - 40 a year. Also does veinous leak surgery.
Hegarty was really nice and seems like a great guy, understands the trauma of ED.
The only thing that put me off was, at the start of the consultation when he was examining me, I was lying flat on my back and he took out a measuring tape and said we'll measure your stretched flaccid. He gave my penis a pull behind the glans, but did not stretch it aggressively to max capacity. He measured 12cm from base to tip.
Later we spoke about implants. I asked him how he sizes patients, does he pump the penis with saline water for surgery, he said no. He just stretches the flaccid penis. I asked does he give it a good hard stretch, he said yes. He said he also pushes the dilators right up into the penis.
I asked do his patients lose much size with the implants, he said no not really.
I have decided to try the veinous leak surgery with him 7th April, he said it has a success rate of about 1 in 3. If that doesn't work I'll probably do the implant with him, but I'm just concerned he will not size me right. When he did the flaccid "stretch test" today it was not a maximum stretch, and was a good deal shorter then a full stretch. This is my biggest worry.
What do you guys think of this today? (He also advised me to check out Franktalk lol).

The venous leak surgery hasn't helped anyone in my knowledge. Lots of guys it made things worse.
If you're going to let doc cut open your penis, why not just go with the implant? Success rate is close to 98%.
Yes, the stretch test is what lots of docs do. It is not the most reliable sign of what you'll end up with. The only way to accurately measure you is after your corpora cavernosa is drained & removed. Cycling after the surgery is said to help regain length.
My opinion- do not do the venous leak surgery.

I would not bother with the vein ligation procedure either. I haven’t seen ANYONE on here report that it worked and solved their problem. Not one person.
Regarding measurements. The most important thing he said is that guys don’t lose much size with him. That’s good news.
The only way to measure for implant size is the dilators he mentioned. Once he has made an incision in the corpora he will use dilators with measurement marks on them from that opening into the base of your cruz inside your pelvis and again from that incision all the way into the glans of your penis. He’ll add those two numbers together and voila, THAT is the size of the implant. Different men have different depth cruz. That’s why an external measurement can’t tell the size of the implant.
Ask if he can put you in touch with any of his younger patients. My surgeon’s office provided me with a list of patients that were willing to talk on the phone about their experience. That was really helpful. I was able to talk with them about what to expect in the recovery period as well as their satisfaction with the results.

Interesting. I was surprised he offered the veinous ligation surgery because it seems rare. I just thought I'd try it as a last ditch effort to see if it helps, before making the end stage decision of an implant.
I can still get erections but can't hold them, so my thinking is maybe if he ties off one or 2 leaking veins I'll be in great shape because I'm not totally impotent.
Telling me that most patients don't lose size was good, but the way he measured me for the stretch test worried me.
Maybe I'll consider skipping the veinous leak procedure, he said it has a 1 in 3 likelihood of success, and maybe I'll go straight for the implant next month. Like others said, what he did in the office is not how the size of the implant is determined. There are two measurements, distal (outside) and proximal (inside) - and there are other considerations. Knowing that, maybe you should revisit that part of the conversation with him to get a more detailed explanation. That may make you feel more confident about the implant option and in he as a surgeon.
