20mg cialis (Tadalafil) is a pretty big dose and it is not giving you great results. Viagra didn’t work either. So oral medications seem to be ineffective for you.
It may be worth continuing 5mg Tadalafil or 10mg Tadalafil daily.
But it sounds like it is time to move on to injections. I you had mentioned a medication that was inserted into the urethra earlier in your post. That is most likely prostaglandin, one of the primary medications in most injection ED mixes as well.
Give those a try, but if they don’t work it is time to move on to implant. I was implanted by Dr Eid and recommend him highly. Dr Kramer also has an excellent reputation. I would go with either of them over a local doc that doesn’t do as many.

At the risk of sounding like a "broken record" (old term, lol), I have to ask if you've tried pumping up with a medical VED, and if that worked. With such a device, if the erections you create does not stay hard after you remove the pump, you can apply a constriction ring directly from the tube to keep the blood in your penis, for intercourse or whatever. With some practice and patience, this method works for many men. VEDs are very safe, when used correctly.
I'm in my early 70's. Serious problems with ED started in my early 60's. ED drugs were eventually not effective enough. Aquired VED pump and constriction rings in 2024 which provides usable erections for sexual activity.

Hi Kimball. Hope I am doing this correctly. My Urologist recommended I try a VED. Picked one up and was disappointed. It would suck up my flaccid penis. It never got hard so a VED for me was no good.
Thanks for your input.

That's weird. Was it a medical quality VED made by a reputable company, such as POS-T-VAC, Encore, Augusta Medical Systems, and a few others?
