Hello again brothers. Not a day goes by that I don’t check this forum. My married life has improved after a 13 year period of my checking out of the marriage. Intamcy between me and my wife has returned. It returned after my awakening of my wife’s serious stays in the hospital in autumn of 2025. Kidney stones and blood sepsis. We went on a cruise in May and twice I received oral during the cruise. I was surprised that my limp dick could achieve orgasm and ejaculate. My wife refused to let me perform oral on her, so far no go for me to perform oral. We are still working on that, But she is ok with oral on me. Many times after the cruise, I am a fortunate man. We are waiting for her cataract surgery on both eyes in Oct. and Nov. I have a surgery with Dr. Kramer planned in January. Have not met with him face to face yet. Not sure how this works after initial telephone consult. Maybe some of you Kramer patients can give me an idea of the process. When I meet with him face to face I will discuss an IPP and MPP implant. At age 74 I don't have many years left on this earth. Not sure an IPP is a good way to go. thinking a MPP is a better idea. A one and done situation with no revision surgeries. I have viewed many videos on YouTube regarding IPP surgery but am struggling to find many videos on MPP. Is there a certain brand of MPP that you could recommend? And how about a Dr. that does a lot of MPP operations, So far i have found Rigi10 and Genesis. What are your experiences and are there other MPPs i have missed. I need to remind you that I suffered a traumatic brain injury (TBI) back in 2012. I easily get confused and forgetful.. I keep a daily journal on my IPad to help my memory. I am on Medicare and Federal Blue Cross Blue shield insurance. I think we also are on a Medicare supplemental insurance policy due to my TBI,
Well that is about all for today. I look forward to your responses.

Re: IPP or MPP?
Both IPP and MPP have pros and cons. MPP is more prone to erosion since it’s hard all the time, and that can be a bad scene if it happens. It’s also a little more mobile so the erection might seem a little less natural. But it’s definitely maintenance free.
59yo Coloplast Titan 28cm Penoscrotal with Dr. Hakky 10/21/2025.
Pre-op erect measurements:
8.5"L and 6.4”C
Post-op: 8.9” L and 6.2” C at 10 months

Re: IPP or MPP?
IPPs are work intensive, significantly more prone to failure than MPPs (that's just common sense: one is a complex 3 piece device + tubes, the other is a simple 1 piece device) and IPPs need to be revised every so often. Doesn't mean that MPP can't fail but the risk is much lower than with IPP.
American doctors would usually try to sway you towards IPPs. First, they have significantly more experience with IPPs than with MPPs, second, IPPs are significantly more lucrative for them than MPPs, especially due to the IPP's almost definite future revisions vs. MPP's likely absence of such, and IPPs also have more natural looks and are easier to conceal.
MPP on the other hand is just less work intensive and is ready when you are. Lift it up and go to town with it. IPP you have to pump up and cycle. I'm told that sizing up MPP involves more fine work than IPP, which if you think about it sort of make sense. One is connected to its other parts with tubes which probably provide at least some additional stabilizing support. The other is literally hangin in there. Size it too loose and it gets wobbly, too tight and it gets harder to conceal and uncomfortable. I also heard that there is a difference between when doctor sizes up MPP as a temporary place holder before you're ready to swap it for your IPP, and when MPP is sized up for permanent placement. I came across 3 people already that got their MPPs as a place holders but liked them so much that when it came time for them to switch to IPP they just decided to keep it.
I have a virtual with Dr. Shaeer coming up who has ample experience with both MPPs and IPPs. Will pose this questions to him and see what he says.
Don't expect any length or girth enhancement with MPPs. In fact I heard that MPPs are customarily undersized girth-wise to minimize chance for erosion. Some people but not all do lose a bit of girth, but I'm yet to come across anyone who lost length.
Here are Shaeers videos of Coloplast Genesis, his preferable MPP:
https://www.youtube.com/watch?v=1opHY5g7LNw&list=PPSV
https://www.youtube.com/watch?v=GR4rFeoHGG8&list=PPSV
https://www.youtube.com/watch?v=XB0pcl0-mYY&list=PPSV
Gives you a good idea of what to expect, at least as it pertains to Genesis.
There are 3 types of MPPs on the US market (I think there are 2 more in Europe).
Coloplast Genesis - the most concealable, the most customizable to your size, but the least rigid.
Rigicon Rigi10 - more rigid and I hear all around better looking. I talked to 2 people that complained about its spring back though.
Boston Scientific Tactra - the most rigid but the hardest to conceal.
Google Genesis vs. Rigi10 vs. Tactra. That should give you a good overlook of their pros and cons.
There is only one higher volume MPP implanter in the US, Dr. Rafael Carrion in Tampa. Everybody else are high volume IPP. All implant MPPs from time to time but not often. From what I hear outside of the US it's 3:1 in favor of MPPs.
I personally have been considering implant for about a year now, and still have few more months to go before I'd be able to go through with it. I was always thinking that I'd get myself an IPP but lately began strongly leaning towards MPP. Thinking Genesis since I don't care if it's the most rigid. I just care for it to be "rigid enough." So far I didn't come across any complaints in terms of that in relation to Genesis so...
Rigi10 is also in the picture. I have appointment with Dr. Carrion scheduled in early December. Doesn't mean I'd end up going with him though because there is a lower volume implanter, Dr. Martin Gross, who is more local to me and who I like and trust very much. Gross has scant, but still only positive feedback on FT and Reddit.
The day before I see Carrion I hope to have an in person Show and Tell with one of his Rigi10 recipients. That should help me settle things.
Dr, Kramer is a very competent implanter who is primarily known for his painless recoveries. I think he is the only one who is able to claim that since too many of his post-ops mention it on FT. Some other doctors might give you a 3 days nerve block for immediate post-op but then it wears off. Apparently Kramer is actually able to achieve pain free experience throughout, and from what I understand without using any medication. Doesn't mean that he implants many MPPs though, but you should be in good hands. I hear his favorite MPP is Rigi10.
It's basically better concealment, more natural looks, more control, generally higher satisfaction rates, and less risk of erosion of IPPs vs. no extra work (re: no cycling & pumping), no revisions, and significantly diminished chances of the device failure of MPPs.
Pick your poison.

Re: IPP or MPP?
At our age (I'm 75), I don't think there's an easy answer. Do to a complication of my original surgery for an IPP, I ended up with an MPP for over 3 months. I hated every moment of it. I could bend it as I wanted to and have sex towards the end of the 3 months, but for me it was a miserable experience otherwise. It was very difficult to get it placed in my underwear/pants so I didn't have a lot of irritating rub. All that said, I know many in FT have them and are very pleased with them. The individual experience is all over the place, so no easy answer.
On the other hand, I have an AMS 700 CX and 10 months out I'm already thinking of a revision. My CX works and we've have many pleasurable sessions. But, the CX has folds near the glans that causes enough discomfort that I have difficulty in achieving an organism. I've had one discussion with the surgeon about possibly going with the LGX and I now have an appointment with a high volume surgeon to get a second opinion (and if she recommends the revision with the LGX I'll likely go with her).
No question at our age choosing either option can have positive or negative outcomes. (FYI, pumping and cycling is no big deal as long as you don't have any issues with your hands).

May I ask what MPP did you have? This is important for me. PM please, if you feel more comfortable.
LGX is a bit controversial and not recommended for people with longer length instruments and those that already have a bend because it would (almost certainly) make the bend more profound.
That being said I know one LGX recipient that says that his bend makes his SO happier. It sort of acts like a fake girth enhancer thus creating additional pleasure from the girth-like experience. Apparently, if your wife likes them girthy the bend wouldn't create a problem (for her) but rather turn into asset
Most LGX recipients that I PMed with (and there were many) seem to be happy with the results. Many praise its length expanding capabilities and natural feel. I spoke to two that even revised from Titan to LGX which made both them and their wives significantly happier due to that "natural feel". I myself was considering LGX only, before I began leaning towards malleable.
Nowadays however many LGX recipients would shy away from discussing their experiences in public because it could get pretty contentious with some of the non-LGX recipients. Sad, but true. That's why you no longer have many of them discussing the particulars of their experiences on the open forum.
Also, as you probably know by now, many implanters have a tendency to shy away from LGX. Many wouldn't implant it if you're over certain length pre-op. Primarily I think because of its bendiness, issues with rigidity for longer penises, and because its length-enhancing capabilities increases risk for erosion (which sort of make sense, eh?).
That being said, for example, Hakky would implant LGX (that's who I was thinking of implanting with if I went with IPP) but in one of his comments on Youtube does refer to LGX as a rotten implant (or something like that). Clavell is even more strongly anti-LGX, although I know a member whom he placed with LGX few years ago (he says he literally had to beg Clavell for it) and who seem to be very happy with the result. I hear Levine makes you sign some waver of responsibilities before implanting LGX, and so on. you don't even want to know what Eid thinks about LGX, trust me.
Among high-volume implanters Kramer is probably the only "anti-LGX" implanter out there.

I had the Boston Scientific Tactra before the IPP surgery. With the implant, I was supposed to get the LGX, but during the procedure he became concerned about a floppy glans and switched to the CX. During my last appointment, he kind of hedged his bets and said that maybe the LGX would solve my discomfort situation. Not sure why the floppy glans situation isn't in the picture now, if it was then. That's why I have an appointment with a high volume surgeon for another opinion. This surgeon is a Boston Scientific Center of Excellence doctor and has done high volume work for a long time, including a lot of corrective work.

Tactra. That's the problem imho. Not MPPs in general. If you look into it you'd find out that it's the most rigid, the least flexible, and the hardest to conceal among MPPs that by default are considered hard to conceal. I was all pro-AMS and anti-Titan when I was thinking of IPPs but that turned 180° when I began looking at MPPs.
Genesis is the easiest to deal with in terms of how it related to your body and the least rigid. Talking to Genesis recipients however you get a distinct feel that it's "rigid enough" for any penetration, and just today Genesis recipient on FB told me that it feels completely natural. Which is what you want imo. You don't need rigidity for the sake of rigidity, you need it just to be rigid enough to penetrate whatever pussy you're set to penetrate.
As for Boston Scientific (or Coloplast for that matter) Center of Excellence... don't let it sway you one way or the other. Unlike Rigicon, both Boston Scientific and Coloplast are not penile implant companies but medical device companies. Penile implants is but a small part of what they do. Center of Excellence is given for utilizing company's medical device output in general, not for penile implants.
As far as I know Boston Scientific's highest calling is to try to help Elon to get to Mars. How much do you think penile implant devision factor in that equation? Coloplast is "a prominent Danish multinational medical device manufacturer."
The question you have to ask her are (imho) - and watch her face to see how to see how she responds because many doctors embellish their numbers like crazy!,
1. How long you've been implanting penile implants?
2. how many penile implants you placed in your career? They all know! All of them, no exception! If she somehow appears evasive - run!
3. how many of the type that you're leaning towards (in your case it would be LGX)
4. how many in the last year.
Then you make an educated decision whether you want to take a risk with that surgeon. Because the risk is yours and they can always blame the device malfunction.
Just my two cents.

Re: IPP or MPP?
Thanks for the replies brothers. Like I stated before I would just be happy to get a hard on. It’s beeen 10 to 13 years for me. My memory not too good. I know that since my wife and I have gotten intimate again, meaning oral for me, that even though I take 200 mgrm of Cialis, I only get a semi erection, a ‘chubby’, I’m not concerned about length or circumference, I’m just looking for an erection of what I have now. Anything would be an improvement. At my age, 74, I don’t care if I go IPP or MPP, I just want to make love to my bride again. Maybe I’m just dreaming and not facing reality. All the more for a sit down face to face consult with the implanter. Anxiously waiting for face to face consult with Dr. Kramer.
