If you want to "cure" your ED then you need to identify the underlying cause. Blood tests can identify hormonal problems that might cause ED (your doctor might order something like a "Men's Wellness Test" which checks various hormones, lipids, etc). Penile Doppler Ultrasounds can assess blood flow in and out of the penis and also identify physical abnormalities such as scar tissue/plaque.
Also, you mentioned the use of supplements. Some guys are convinced supplements like horny goat weed or maca are the real deal. But IMO these are the guys who don't have ED so the supplements just have a placebo effect for them. I tried tongkat ali for a while and it didn't do anything for me. Same with those dumb "Libido Max" type products you see in pharmacies sometimes. After that, I made up my mind to only go for proven treatments that are backed by the medical field. I'm not saying that there's no supplement in existence that might help you, but personally, I didn't want to bother wasting my time and money anymore.

You are right, by far most ED treatments are symptomatic. They temporarily help you achieve erections, but they do not address the underlying cause of the ED.

User caddyman from an online health forum had venous leak by birth and got an implant sometime in his late 20s/early 30s. He reports back in multiple posts indicating he is happy.

Okay, so I realize this turned into a post completely about implants which is probably not what you wanted to hear. Some men have bad experiences with implants and you need to consider that too because the surgery is irreversible. I admit that I am biased because I want one. So do your own research. You should find the underlying cause of your ED and treat it if you can. But if you find that your ED is in fact permanent then the implant is there for you. The idea that there are "no options" out there is, thankfully, not true.
