I'm pretty surprised with all the replies! Thanks so much everyone.
I think I'm going to try another lap of all the treatments again except the shockwave which I think I've exhausted.
I'll push for another NPT, Doppler, Hormone Test and I'll revisit the tadalafil and sildenafil pills just to see. Also the highest dose of MUSE and Vitaros.
Im curious about Trimix and Quadmix but as they are "in the moment" medications they don't offer me much than what I currently have in the Alprostadil which works fine for me.
I haven't mentioned Invicorp yet either. If there was a local drug I could take daily that would give me spontaneous erections like tadalafil then that would be the ideal instead of immediately before sex. But then I'm also getting tired of having to source prescriptions and obtain medications (and make sure I don't forget to bring them on vacation)
The Vascular surgeries now seem to be the only final treatments left for me to explore before pulling the implant trigger.
If I'm honest I'm kinda excited about the implant because I know it's guaranteed and to bring to an end a lifetime of penis and erection problems and anxiety is very encouraging.
But just what if there was something out there that I've not tried that could cure me....
Also there are some concerns I have with the implant. Im very much a grower so I'm worried about my glans not matching my inflated dick, I'm worried about the resevoir. I do martial arts and I'm concerned about impacts to or pinching / crushing of the resevoir etc.

If none of the previous treatments solved anything why repeat?
You're chasing a cure that isn't going to happen without a miracle. Time is your enemy as size loss happens when tissues aren't stretched. Lack of erections can lead to shrinkage.
Martial Arts may be something you have to give up. A kick to the groin or reservoir may be harmful. That choice makes this decision tougher.
Your glans have a separate blood supply and should engorge as well as they do now. It will take some time following implant for all the sensations and engorgement to return.
Don't overthink this.

Yes, I do realise that there is a very unlikely chance that I'll find a long-term alternative to an implant. However vascular surgery in particular is something I've not at all looked into and as this is a final decision I need to make sure I've exhausted all I can psychologically.
Stopping martial arts is a big one.... That's a big blow. I'll have to look into this more and consult Prof Ralph on it. Perhaps another reason to delay. Only thing is I'm only wanting this to be performed by David Ralph and I worry when he's thinking to retire.
Erection wise I get around five per week. I sometimes wake with erections and I still stimulate it myself. The problem is they die within less than minute. I'll start using the pump and extender again to counter this.

Get a injector called Inject-Ease and you excuse yourself for 1 minute to a separate room and be ready to go.....so easy
Also,a big plus is that you will be able to last 3 times longer than her last partner...she will be amazed....
Yeah I do already have good results from Alprostadil already but I'm currently single and I'm looking to progress away from "in the moment" applications. Even my ex expressed disappointment when I would interrupt intimacy to go and inject.
How long have you been using the Trimix? Did you have any issues with peyronies as mentioned above?
I started using using Trimix a little over a year ago.
I've had a bit of "curvature" for as long as I can remember. As best as I can tell Trimix has not made it worse.
