Legs are shrinking fast - low T or a blood flow issue?

Hi Guys,
So I have posted on here a few times but I would love some feedback. I am only 23 years old and my Primary doctor and endocrinologist insist that I do not have a testosterone issue because I fall "in the normal range" even though in 2017 my reading was 488, in 2018 it was 350 and in 2019 it was 250. After voicing my concern, I got mine checked again and the doctor told me it was up to 600 all of a sudden which confused me honestly. Does it vary that much day to day? Regardless of what they say I know that 250 is low for anyone, but especially someone as young as me. But they insist above 200-1100 is "normal". I still think this could be the issue but I tried to move on for a while. So recently my friend was taking some candid pictures of me and I was appalled to see how much my body is changing physically. The following are the symptoms I am experiencing:
Feeling:
Dull ache in legs/knees (could not sleep)
Dull pain in testicles
Dull ache in lower back (could not sleep)
Urinary retention and pain
Cold hands/feet
Erectile dysfunction (no morning erections at all)
Extremely premature ejaculation (chronic)
Fatigue
Visible: (pictures to prove it)
Increase in fat around waist and breast area despite regular exercise
Decrease in muscle/bone in legs and arms
Weight gain in face
Rapidly receding hairline
I am wondering if people think this is Low T or maybe a blood flow issue or anything else. If someone would be so kind as to send me a private message I would be able to get into contact with you and send you the pictures to take a look and see what you think. Any and all insight is very appreciated! I put them together in a collage to show the difference from 2016 to 2019 side by side and I think it is pretty significant. I exercised occasionally in 2016, but actually exercise MUCH more regularly now, so the changes are not from lack of exercise. My legs are so much skinnier, I have gained fat in my face, love handles, and breast area. I just need to know whats happening and I need a doctor to believe me. HELPP thank you all so much.
