As one who is finding Sildenafil becoming ineffective after two years, I found it interesting that this site recommends prescribing Cialis first --- i.e. favored over the alternative drugs.

Cialis is an excellent treatment and it was my #1 ED drug for more than ten years. A few months ago, however, it stopped working for me - I can't really explain why. Today I'm taking avanafil (brand name Spedra) with excellent results: it is quicker to act, has fewer side effects and the effect lasts for several hours.
Conclusion: Not all PDE-5 inhibitors are the same, reactions vary a lot.

I found that Viagra worked best for me. I started with 25 mg and over three years worked up to 100 mg. I still get a slight plumpness from Viagra but nothing from Cialis.

The range of PDE5 inhibitors currently available have slightly different actions in the body and selectivity for inhibiting phosphodiesterase types plus different half life's in the body.
Cialis is spoken about so much because its half life is so long compared to the other and therefore provides more spontaneity. If it is taken in a low dose daily it offers an almost consistent boost for erectile function 24/7, so the user doesn't have to think about when he needs to take the drug other than it being routine. This I feel is a great advantage to the others, if the drug is effective. Not only does it offer the "on demand" effects, but science has discovered that PDE5 inhibitors may help to keep the cardiovascular system in better shape by combating oxidative damage and preserving endothelial health if used chronically(daily). Therefore Cialis makes good sense here as it has such a long half life.
Having it in the body every night to help assist nocturnal erections is a big bonus as more oxygenation in the erectile tissues will occur, helping to stave off the effects of hypoxia.
The effects of hypoxia in the penis are the focus of much research in regard to ED.

Very interesting Simbarn. For starters, after a telemedicine call with my doctor, I decided to start with 10mg tablets which I'll in batches of two "as needed" ( e.g. Friday night) But I'll consider the daily advice.
I never really thought about "maintenance" and exercise of those body parts. I've started looking at Kegel exercises. Makes sense that muscles etc only used once a month might suffer, especially as you enter your 70s.

I have been taking 5-10 mg of Cialis as needed the last 10 years. The side effects are not fun and getting worst. I tried 5 mg daily for a while and had to skip some days as side effects we getting too bad.
Tried higher dose and its not better and some case its worst with more side effects of stuffy nose and sour troat.
The last week I have been cutting them in smaller 2.5mg pieces and taken daily before going to bed.
Im getting Daily morning wood, stronger and harder erections I have not seen in a while.
Side effect are lower with the smaller dose.
Will try this for a while and next time may order some 2.5 mg size instead of cutting them.
In the case of Cialis Its a weird drug, more is not better as I have seen the bigger dose achieved lesser results.
