Diagnosis of Severe ED - Pelvic Floor Dysfunction!

Hi folks, just wanted to post an update regarding the diagnosis of my severe ED symptoms.
I pretty much have been feeling lack of sensation / nerve response that should normally bring about erections, even though libido has always been high. I had consulted with other urologists in FL with no proper diagnosis or insight into what could be causing this issue. After all, the results from the total blood/hormone/urine tests only demonstrated great overall health and levels, and the results from the penile doppler study also showed great blood flow (no evidence of venous leak). I also don't have any history of major trauma to the pelvic region, so possibility of nerve damage is almost close to none. Even taking daily cialis (10-20mg) hasn't had any noticeable effects (or side effects for that matter).
However, after recently consulting with one of the top urologists for ED in Florida, I was given a clear and prompt diagnosis of severe pelvic floor dysfunction (after discussing my situation and having DRE/prostate exam done). He basically explained that my pelvic floor muscles were constantly in a state of spasm. He's now suggesting professional physical therapy to relax the pelvic floor muscles. I agree that it makes sense that this has been the root cause of my problem all along, and I hope to hear from some other folks on here who have been or are still going through the same situation with regards to pelvic floor dysfunction and how effective the therapy can be.

It is excellent to hear that an actual urologist diagnosed you with a pelvic floor issue.
Some are seeing this as a valid condition. Progress!

Hi abhishek26,
All my symptoms, results and even the treatment (Cialis daily) are exactly the same as yours (perhaps the cause is the same as well?). It's possible that this condition can be a legitimate stand alone reason for ED as well.
I myself (and perhaps other young men with the same condition) will be interested to read how your treatment progresses. Please update this thread with your progress.

@EveningWood, sorry to hear about your situation as well, although it is very interesting to hear that your symptoms and conditions are very similar. Have you by chance tried injections yet (as that seems to be our way out of this hell in case therapy doesn't improve the situation) ?

Funny...been to several urologists now....none of them have done a DRE to assess my prostate, which I find odd. Do you know what the doc felt in terms of physically to determine you were in a constant state of spasm in the pelvic muscles?
Sidenote: I guess Prostatitis is a real thing that isn't always caught with blood/urine samples....apparently it's caused by bacteria (some have even suggested it's from a form of STD)....and the typical short term antibiotic cure helps at first but comes back after the antibiotics are ceased. Some of what I read suggested courses of antibiotic treatment for 3 months plus. Obviously, there's inherent risks there as well.

I want to start gradually and try PDE5s first. As far as I've seen from the forum, injections have a relatively limited period in which they are effective. (most members take for 1-4 years, before the effects decrease or disappear). Also, I want to have sex more that 2-3 times per week, so trying to find potential solutions helping me with that.
Sean762 - I have thought about that as well. But in this case, won't the enlarged prostate be visible and obvious on the ultrasound check up? Therefore any urologist/general practitioner should be able to tell. Myself for example, did not have any changes to its size. As far as I know, an enlarged prostate also causes night-time trips to the bathroom and difficult urinations as well (symptoms that for example I do not have).

@Sean, yea he basically did a DRE and immediately detected that my pelvic floor muscles were extremely tight (not only that, but he also noticed that my muscles were constantly locked even when I tried to relax them, hence the spasm). He didn’t see or feel any issues with prostate or bladder, even though my previous urologist had misdiagnosed my case as prostatitis and had given me antibiotics and cialis to try (which were of no help in my case)

I have pelvic floor issues as well I believe. I go to a pelvic floor physical therapist and it seems to help a little. Although nothing drastic in change yet. I’m having a cystoscopy this coming week which I am not looking forward to but I have a feeling the urologist won’t find anything. He does have me on injections since the pills don’t work all that well alone. I usually do a small dose of both and that seems to be most effective for me.

@mark, yeah I saw your recent success with injections. lol congrats!
really hope pelvic floor therapy resolves my issues, but, if not, I know injections will probably come to the rescue

The Doppler U/S is just on the base of the penis for blood flow...it won't catch an enlarged prostate.
Yeah, the accounts I've read were the person had prostatitis did not mention any issue with increased frequency or difficulty urinating....just ED.
