How quickly is Venous Leak deteriorating?

Hello everyone. Started to experience VL symptoms 7 months ago after a COVID (it was mild tho), when I noticed that my glans looks soft when I lay at my back, though it's ok when I sit or stand. At the same time I started to feel some pelvic pain, irritating to different parts of body. 3 urologists said it's psychological, but they haven't even done any examinations. Recently I did a Doppler ultrasound test and it showed my dorsal vein is leaking, but I think big fear and anxiety could affect the results.
Anyway, I started to take Tadalafil 2.5 mg every day, my erection became better when I sit or stand, but when I lay, it seems like pills don't help enough to make my glans as good as it was before COVID. Honestly I'm frightened because I'm afraid that pills won't work at all with my VL so I ask you guys, especially who has a VL, how much time I have before my ED become severe? How fast VL progresses?
Btw it seems like raising the dose to 5 mg doesn't change a lot, but my nocturnal and spontaneous erections are getting even annoying
