Hi everyone, first submission here, I hope I’m posting it in the right place.

So after years of struggling I finally managed to get myself properly checked by a doctor. He diagnosed me with a severe case of venous leakage after checking me with a doppler (genetic vascular anomaly as most probable cause). He recommended surgery right away, a so called DPVL (dorsale penile vein ligation). He said he performed this kind of surgery thousands of times, that he sometimes had to repeat it once, but never twice.

Now, hearing him talk I should be good to go after surgery. But what I read around ranges from disheartening (success rate of about 55% in optimal conditions, though the paper is from 2003) to downright terrifying (“Perioperative complications include wound infection, penile curvature, skin necrosis, painful erections, postoperative shortening of the penis, penile deformity, transient or irreversible numbness, lymphedema, and ligation of the penile artery by error”, I mean holy shit). Most of the stuff I read online seems to point to the fact that this kind of surgery seemed promising at the beginning, but it has been now abandoned in favor of “other treatments” (penile implants being about the only one I was able to find).

Has someone in here undergone this kind of surgery? Or has had the same diagnosis and has been advised against it by his doctor? Is there anyone that managed to fix his venous leakage somehow? Any experience to share at all?

I got my appointment in about a month, and I’m really torn if I should do it or not. Worried it might do more damage than good. Pretty scared overall. I’m 39 and otherwise healthy.

I have no knowledge on the subject but I think I've only ever seen one positive post on the procedure here on this forum.

I'm about your age, and going through similar issues. I'm currently trying to find a good urologist who can order me a doppler ultrasound to diagnose/rule out venous leak. If I were in your situation, and offered that surgery, I would probably go see another urologist from a different office. A second opinion might help, because like you said, you don't want to make a bad situation worse. More information might help you make a better informed decision.

I've only heard positive things about embolization, but never about ligation

I might sound like a shill for implants, but is that something you might consider? I had pretty serious venous leakage and of course the implant took care of that for good!
Just my very non-medical view fwiw.

How old are you ? Are you able to achieve erections and have penetrative sex at all ? Even for short amount of time ? Are you able to do it without PDE5 pills ?

And how does erection and intercourse looks like sober and with assistance of PDE5 ?

do not under any circumstances perform this operation. it will only get worse!

I agree with what everyone else has written above. Penile vein ligation has been out of fashion for 20 years, because it doesn't work, and exposes you to unnecessary surgical complications, including penile scarring and shortening. At the very least please get a second opinion before proceeding.

I just had implant from not being able to hold an erection. Not entirely true, I could get and keep an erection but not strong enough for penetration, only for manual/oral play. I just started cycling and I can tell you that my penis feels pretty much the same as back in the good old days. It reminds me of my hip replacement it is all I thought about for awhile, now it is just my hip. Lets me walk lets me dance, crouch, crawl, all that stuff.

Same with my penis. I can see after only a few days how it is pretty much the same penis I had years ago. We are using it tonight and my wife can't wait. Also, I went from shower to grower with the surgery and I do not mind the bulge at all.

Good luck whatever you decide, but my doctors impressed on me how important it was to get an implant correctly installed. It was the only thing that actually works to fix ED permanently if you cannot hold an erection, regardless of the cause. My doctors did not diagnose me, they just fixed it. I will take that any day. 98% patient satisfaction? Why swim upstream?

What is your opinion on embolization ? Is it worth checking ?
After hearing gollam story im terrified when it comes to any kind of operations.
