Diamond in the rough days

Long time reader/First time poster here. I have been struggling with ED for several years, but has been worse the last couple years. Just on 5mg daily cialis for the past 6 years or so, but took tons of on demand Viagra/Cialis for years before that. Most of the time, I feel very little effect from just the daily 5mg cialis regimen and will often supplement with either another 10 mg cialis or 50 mg viagra on top of that if sex is in the cards. Even with the top off, still never feel like I get fantastic erections.

On rare occasion(2 to 3 times a year), for whatever reason, I will have an amazing day where I can get a diamond cutter boner on demand and fuck like a pornstar on just the 5mg Cialis. Today was one of those days and while it is great, it confused the hell out of me and makes me wonder why it can't be like that more often. My wife was pleasantly surprised as well and said "Where the hell did that come from?!"' when we were done and I do not know myself. Most of our sex lately has sucked ass where I was 70 percent hard at best and just stuffed it in there and hoped for the best. I find when I have an iffy/unreliable feeling erection that is what works best for me, but obviously not ideal for her. Tonight I could do anything.... started slow and shallow like she loves then moved to hard and deep at the end. Overall best my dick had worked in several months and glad it got put to good use.

Anyway, Today was no different (better or worse) than any normal day and still have same level of stress, etc. Anyone else here ever get these "diamond in a rough" kind of days?

I have the same issues. It seems if I slack on the Cialis for a few days then pop a Viagra i will perform better.

My issues is side effect, back pain and headache on Cialis and neck pain and stuffy nose on viagra.
I stop daily cialis for this reason. Mostly viagra now in the 50-75mg dose.

Viagra gives me a harder erection for sure.

There are just so many moving parts beyond what oral meds you have taken - Mood/stress/diet/hormones, erc, etc. I have been on TRT for past year and a half and levels are solid now...900 plus consistently.

I just wish I understood the ebbs and flows. I just go through phases lasting a few days where I will feel like I am 16 again and horny as hell/able to get hard on demand. Then that will end a few days later and I will go back to feeling like a house plant again - no libido, feeling like I can't get it up with a crane and do not even care. Mind you this is all with steady T level due to my TRT so I do not think testosterone is an issue here. What sucks is as I get older(40 now), the houseplant phases are way more frequent and long lasting than the 16 year old me phases.

Could my sex drive really be that low that I only really WANT sex a few times a YEAR? Then all the other times in between, I just half ass it for the sake of doing it and it just is blah. In my teens, I was an avid masturbator - I would go 3 to 4 times a day on average until I was 18 or so... hell there were days where I approached double digits if I were bored enough. I am only 40, not 80, so don't get why my drive/erection cooperation is so low and not sure where else to look other than hormonal stuff that has already been addressed.

Forgot to add in terms of hormones,
all my other levels such as estradiol and prolactin have been fine as well (I do lab every 3 months) so again, nothing hormonal points to a problem.

I just find it hard to believe this is all just a psychological issue, but when I have good phases like the one I mentioned in the first post, it shows my dick IS physically capable of performing VERY well on occasion. It just only happens a few times a year.

Are there any issues such as blood pressure, diabetic, or cholesterol issues that may be in play?
