Erection comes and goes during sex

I'm 48 and the first signs of ED came about a couple of years ago.
I have no problem at all with desire. Its just that my penis doesn't always cooperate.
I can get it up - though its not nearly as hard as it was just a few years ago.
Problem is, I lose the erection at some point while inside my wife.
When this happens, a blowjob will usually bring it back to life and I can finish the job.
But sometimes that doesn't quite help enough.
I never lose it while masturbating so I figured that was a clue. I had my wife stroke me with her hand instead, and found that I maintained a longer, harder erection.
Then I would get inside her again and try to finish the job quickly before it goes down again.
My doc gave me a cialis sample. So far, I haven't noticed much difference with the pills, except she says the head seems more firm and full.
I'm not a heavy smoker but I usually do have at least one cigar per day. From reading up on ED, I know that smoking can contribute to issues.
Are these symptoms of ED indicative of anything in particular?
Is it just a result of 30-something years of masturbating like a spider-monkey?
