I have tried without the insert but it sucks in lots of skin and then also very difficult to remove penis from the cylinder

I have similar problems. I have used a Pos-T-Vac MVP-700 with the large size cylinder for a number of years, It's about 2 1/4" internal diameter and 10" long. Even without the bushing in it I get kind of "stuck" on it when fully pumped, I have to use enough lube and sometimes wiggle it around. I also found that holding down the deflate button helps, otherwise residual vacuum will fight you.
I just got a LeLuv pump (available on Amazon) that has a 2.5" ID cylinder. It's better for me not getting stuck, but my balls tend to get sucked into it (mine are relatively small after years of TRT), so that's a thing. There's no free lunch I suppose!
