At what point does damage occur in priapism?

Hey guys,

New member here. I am wondering if any of you have had erection issues after having an erection for a little over 4 hours? The reason I ask is I have found that 10 units of Trimix works great for me but my erection will last almost 4 hours with it usually subsiding about 30 minutes before or after that mark.

The guy at the pharmacy told me to star trying to bring it down at 3hrs and that if it goes away right at about 4 it’s not a problem and that his customers that have issues are the guys that will just let it go for 6-8 and not do anything to bring it down like exercise, pseudoephedrine, or cold showers. My urologist on the other hand told me to go to the ER at 3 hours which seems very cautious. I’ve also reviewed the medical literature and found this:

“While priapism is usually defined as an erection that lasts 4 hours or longer, physiological changes and microscopic tissue damage inside the penis typically do not start until about 6 hours after onset.[26]”

Other papers on pubmed say 6 but some say 4-6 I think ultimately they cannot pinpoint the exact time in which damage begins to occur.

So I am wondering if any of you guys erections last right at about the 4 hour mark and if so has consistently hitting this mark caused you any issues?

At 3 hours I get my dick to go down a little bit so it’s kinda hanging but not pointing straight up. I usually have firmer erections the week after an injection (my issues is psychogenic and I only inject 1x per week when I’m with my partner) but this week my erections have not been quite as firm so I am very freaked out about possible damage.

In my humble very non-professional opinion you're fine. You seem to have already done some research and found some good information. I'd say you're fine at 4 1/2 hrs, especially if it's already lost some firmness and is drooping. Starting your attempts to bring it down at 3 hours sounds like good advice. Take your boner killer pills at 3 hours. How soon to head for the ER will depend on how long it takes you to get there. If it's not a quick trip then start early.

Not medical advice as I have absolutely no medical qualifications: I have a technique I've used, which has NOT been doctor approved. I've never even remembered to ask when at the doctors office. Well anyhow, when boned out longer than desired, I've wrapped my hands around my dick and performed a reverse kegel while gently squeezing my dick. Please figure out how to do reverse kegels beforehand. Only squeeze gently as I'm guessing you might actually damage the valves and cause a venous leak if you squeeze too hard. But if you squeeze gently while doing the reverse kegel, your dick will go down. It will reinflate if the drug is still active but if you do it several times you will be changing out some blood. Please consult your doctor first before trying this technique.

I have the same issue...even at the lowest dose my erections last for hours and I have to inject the solution almost every single time.

I am having a hard time finding a dose that only lasts a few hours...my ED is psychological.

I have switched to bimix at a low dose as well and that has not resolved the issue either.

Have you had any erection issues after being erect that long on the Trimix?

I’m only up to 15 injections, but have had longer than 4 hour erections a handful of times. I was worried at first but then realized that they weren’t painful. If it over does get painful I’ll start the reduction process but right now I’m living for them four hours. :D
50+ yrs old. married 25+ years. hypothyroid, on TRT. 10+ years ED, viagra, cialis now 50% ineffective. now on trimix 2MG phentolamine/30MG papaverine/20MCG alprostadil

If the erection is not painful then is fine 4 hours erected, in my case the erection was painful after 1 hour, then I put the erection down after 3 hours of painful erection. After that episode I developed fibrosis on the site of the injection that is causing narrowing/dent/fibrosis. The reason is because it was painful
I had a trauma 2 years ago, caused me narrowing, dent and shortening. PRP injections three months ago=worse narrowing, and worse ED. Now I only have two choices: impotent or implant. Born in 1975, in a relationship with girlfriend fir 4 years

Thanks for sharing. I’ve really been wondering about the pain level associated with priapism as some times on Trimix I will get a slight aching but nothing I would consider that painful.

Could you tell me how bad the pain was for you?

It sounds like what you're describing is alprostidal (PGE1) ache. It's pretty common. Many guys experience it initially but with continued use find the ache diminishes and eventually becomes a non-issue. Some guys get intolerable alprostadil ache and have to go to a mix that doesn't contain any alprostadil. Some guys never have any issues with it. It isn't related to any priapism related pain.

Hillbilly, the pain after 3 hours being erected was excruciating, very painful.
I had a trauma 2 years ago, caused me narrowing, dent and shortening. PRP injections three months ago=worse narrowing, and worse ED. Now I only have two choices: impotent or implant. Born in 1975, in a relationship with girlfriend fir 4 years

Thanks guys. After my last Trimix injection my erections have been a lot softer and require a lot of stimulation. Should I just get an IPP or do you guys think it will get better? I had strong and firm erections just two weeks ago without the Trimix now even on I’m less firm.

Should I wait and see if I get better or aggressively pursue treatment? Worried if I wait and it’s a permanent damage I’ll lose girth and length.
