Compounding Pharmacy

OK guys. My local compounding pharmacy has quit making trimix. I guess it's fallout from the steroid contamination a few month back.

I thought it would be a nice idea of we could get a list going of where one can order for shipment.

Feel free to start the list off. Maybe pricing information would be helpful too.

First diagnosed 2000 psa 14 treatment ebr. reached nadir .4
chemical failure 2011 psa 13.1 gleason 8, treatment hormones followed by rp non nerve sparing Feb 2012
psa after surgery 0.00

I have been using Avella (mail order) 877-546-5779 for past year or so. I get four 2 ml vials at a cost of $78. They are approved by CVSCaremart for insurance coverage so my out of pocket is $40.
