I've used a cock ring for probably 20 years. And I continue to use one with Trimix.
After the sex act is done I take it off. The glans and urethra (corpus spongiosum) return to a non-erect state but the shaft (Corpus cavernosum) remains hard.
As far as the hardness of the penis I would note several things. I found that when it's super hard it tends to ache, Partially from the added pressure, of being tight. Also, the position can make it uncomfortable if it's too hard. Ever since I started using Trimix I found I need to use lube. Lube (for me) makes for easier penetration and reduces friction sensitivity.

Wearing the ring behind the boys also serves to reduce blood flow but not overly so. It also tends to lift the erection. I suspect that's caused by the ring pulling down on the suspensory ligament effectively making it shorter. It also puts the ring more out of the way so it's less likely to bother your partner.

A ring that is worn "behind the boys" is, if I understand the meaning, something rather different than what I understand a cock ring to be. Are you aware of any sites that maybe illustrate such a thing? Sounds interesting.
