Ever forget which side you injected last because maybe 2-3 weeks went by between injections? I figure after 2 weeks it won't matter.

Date,
Amount injected.
If it worked or that we had sex.
Location of injection (left/right side).
How long before the erection started to go down.
Notes column, could be anything, noticing bruising, or reactions from sex by either of us.
