Bimix to Trimix jouney off with a thud

After having a radical prostatectomy 10 years I started using Bimix after disappointing results from a penial pump. Bimix gave satisfactory result at 30/.5 (30 mg papaverine/.5 mg phentolamine) when injected at .15 ml per injection. Over the years its potency began to reduce. I had the formula upped to 30/2 and then January 2026 I had it upped again to 30/3. Unfortunately, the last two changes did little to help me secure an erection that was useful for sex or manual masturbation. On that last doctor examination, he suggested it was time to switch to trimix.
I went back to the same urologist expecting to be examined but instead was met by a Nurse Practitioner with an apparent attitude. Long story but I won’t bore you with it. After dealing with their office incompetence, I was able to obtain a script for Trimix 5 mcg/.5 mg/15mg/ml/injectable) (PGE-1 + Phentolamine + Papaverine). What volume of trimix might be standard as my first injection.
Retired. R.P. 2016. Bilateral nerve sparing surgery. Now use .15cc of Bimix twice weekly & anorgasmia, moderately incontinent. Wife no longer interested so go solo with Electro-stim using Erostek ET-312. Now am Type 2 diabetic.

**Update-6-15-2026, still no response from my urologist's office even after an additional call to them. No idea what's going on with them but I guess they don't want my business. I may have to go back to my urologist at UCSF who is 90 miles away.

Any idea what a safe starting injection would like? How many units?

My urologist told me to start with 10 units for Trimix. I did that with no issues but I pretty quickly moved to 15 units for best results.. Over the past the 3 years I have gradually moved up to 25 units.

Maybe you should email your urologist and ask him what does he suggest for your starting dosage ?

Surely they will answer that simple question

Reggieman: Dump the jerks and look at www.healthgrades.com for a closer urologist than 90 mi. They also grade the doctors in several criteria.
Age 81 in 2026. On testosterone replacement due to hypothalamus malfunction. (Attention depressed guys: low testosterone is a cause.) Healthy health nut but ED due to getting old. Like to keep enough cardiovascular ability to thrust for 25 min.

I found what the problem is. When I tried to get the original appointment with the urologist he fobbed me off on his nurse practitioner. I discovered later that her training is family nursing and not urology. She did know jack about ED meds such as bimix or trimix. She sent the prescription request to the pharmacy without telling them the volume of trimix for each injection. I complained enough and met with her again. She still did not understand the problem. She would think of a reason to leave the room and would be gone for quite some time. Then she came back and spoke to me in a way that was obvious she had been out researching trimix on her computer. When asked questions about dosage she would speak in general terms such as minimum of 10 units and max of 20 units. I told her I had called and spoken to the pharmacist who advised me the formulation for the trimix in the prescription was weaker than the bimix I had been taking. I had told the N.P. I needed trimix that was stronger than the bimix. I don't want to start out at something like 30 units if she get me a formulation wherein 10 units might do the job. She refused to give me a starting dosage in writing and always changed the subject and or left the room for some other reason. The meeting was almost 1 1/2 hours long and she was gone for probably 15-20 minutes during that time.
She finally told me she called the pharmacist and ask her to mail me a paper that explained the dosage. They said they don't normally do that. Today I got a text from the pharmacy stating they were working on a prescription for me but put it on hold till
they get clarification from the issuing prescriber. The N.P. didn't mention a new prescription being requested and don't believe one has been made. She is bullshitting the pharmacist too. I have never seen a professional weave and dodge like she did. I finally just let her sweat it out trying to dodge answering my questions. She just could not admit that she didn't know what the fuck she was talking about and was over her head. In our area there is a big shortage of urologists. I found it strange that no doctor in the office offered to bail her out. I complained to enough people before I last met with her.
