In a completely flaccid penis of a "grower", the cavernosa target is the size of large spaghetti, maybe 1/8" dia. And it's limp and moves around making it hard to zero-in on and penetrate. I've had more misses with flaccid injection than I care to remember because they always cause painful red raised areas. So I inject after aroused and about 1/2-2/3rds the way "up" or so. The target is bigger, and firmer and it doesn't move around like limp spaghetti. You need to feel the needle penetrate the outer sheath and then penetrate the cavernosa wall. Hitting too low hits the urethral/nerve bundle spongosa - not good. Hitting it on the way up - not too aroused - IMO also allows more chemical to "wash in" to more of the cavernosal tissue while filling up.
The bad news is that after injecting large doses for 5+ years, some scar tissue has formed (no peyronies) making it harder to get the needle in, so I've had a few more misses than before and and really having to be careful and drill it in a bit. sometimes it just depresses the cavernosal wall and does not penetrate. Either that or I got dull batch. Of course I use a 1/2"x30ga needle but will go to 29ga to avoid bending them which has happened more recently.
As to bldoink's comment on rings, I like that and have tried, but I have yet to find the size and elasticity ring to help and not cause a sore cold bloated hinged zeppelin that is actually too big for my wife. I have a reject box also. Too hard, too big, too small, too fat, too narrow. The natural injected girth is great (with some recreational VD) but sometimes not stiff enough long enough. But I like the idea of using less chemical with a suitable ring. It may also hold the chemical in better. So I need to find more rings.

NOT an MD. 72, M52 yrs, CenTX US. Inj since 12/2016, a yr after pills stopped working. Caverject for a yr. 1/2018 Tri-Mix at 30 pap/2 phent/60 pge @0.3ml, now 0.5ml 80mcg/ml PGE1. DE/Anorgasmia setting in since 5/2019, worse now.
