I haven't travelled with Trimix, but I did travel with Viagra.
I had my doctor write a letter detailing the prescription and declared it wherever necessary but nothing happened about it.
Needle's and vial would need to be placed in check-in. From my experience and knowledge, both are generally banned from carry-on now days - sharps and unknown liquids. So you'd have to check it in and carry with you the letter from the doctor.
Some countries like Japan have very strict import rules on Sudafed, so be careful on that. However I've not read of any import rules regarding the ingredients of Trimix.
Make sure to read travel guidelines for the country, if in doubt contact the embassy.

I have traveled overseas with Trimix several times for cruises.
I bought a bag designed for diabetics which had cold
packs for freezing. Once on the ship I got ice for storage.
Strength was fine.

I have taken long trips to Europe and French Polynesia.
For Europe I used an insulin travel pack with small frozen packet. On the longer trip, I took a Medicool travel kit that has a larger solid frozen block that kept trimix cold for at least 16 hours. I most cases, the rooms had a mini-fridge. When they did not I got hotel to put kit with trimix in their freezer, no questions asked. You can find both on Amazon, Walgreens, etc.
Good luck and have a great trip.

When I’ve had to travel long distance away from refrigeration I put my Trimix in a small ziplock baggie. I put that inside a good double walled thermos (16oz or 20oz) and fill it with ice cubes. Sealed shut it’s still been cold almost 24 hours later. That would travel fine in checked luggage. Some people travel with a copy of their prescriptions in case they are questioned about medications in their bag but I’ve never had that happen and don’t worry about it.

Don't put the vial in checked luggage! It can bake in the sun on the tarmac and will be ruined.
You should take the vial in your carry-on. You can put it in a waterproof ziploc bag with a couple of small freezer packs. If it is a long trip and you notice the freezer packs getting warm, just ask the cabin staff for some ice.
People travel with vials of medication in carry-on all the time. Diabetics, rheumatoid arthritis patients, etc. It is allowed and nobody will give you any grief.

+1.
A month after starting on Trimix (May 2019), wife and I flew to Munich and drove to the Dolomites in northern Italy. Best trip ever, and the sex was just a bonus! Trimix held up great.

Do you have security do a hand check or do you leave in your luggage and have it scanned. I was concerned the security scanner might damage the Trimix.

No hand check necessary. The scanner won’t damage it.
