Trauma two years ago after couple of Trimix injections, now I have a deformity ns

I'm trying to figure out how my penis got deformed after Trimix use. Two years ago I went to Summit Male Medical Tempe Az for mild ED. I was taking Cialis and it worked, but after hearing on the radio the commercials of Summit promising amazing erections I decided to try, just something new. When I got there the "Doctor" injected Trimix on my penis and 5 min later I was super hard, he told me to take Sudafed if I was hard for 4 hours, then I left. 3 hours later my penis was still hard with excruciating pain, a pain that I never felted before, so I called him and asked him what to do, he told me to take Sudafed but I couldn't wait for the effects because my pain was extreme, so he told me to inject myself with the Autoinject the antidote "Phenylephrine", so I did. The Autoinject is a device where you put the shrynge to inject, but it seems that with the device you apply more force than manually. After the Antidote my erection went down. Two days later I injected again on the same spot from the last Trimix shot with a flaccid penis, then 10 minutes later developed an indentation on the injection side, now two years later my penis is narrower, dented and I have severe ED.
What caused this?
A) 3'30 hours of a painful erection
B) Injecting the antidote with a force on the erected penis ( it was on the opposite of the dent)
C) The Trimix injection two days later on the side of the first Trimix injection
Now I'm completely devastated by the fact that Trimix suppose to be safe, but I ended up with a deformed penis and severe ED.
