So, I have been using Trimix for over almost two months now with disastrously different results:

Cialis gives me blue headaches and no erection
100 mg Sildenafil gives me partial erections only of taken on empty stomach
56, 6' tall, 280lbs, elevated-but-not-high BP
Am on Testosterone shots every two weeks
Already have a downward curve to dick, indicating either just shape, or possibly some mild Peyronies Disease since youth.

Trimix from (medical) friends
Sept 9 - 5 units = 45min medium erection
Sept 23 - 10 units - 60min medium erection

Trimix - My Own Prescription
Oct 3 - Got my own prescription (Alprostadil 5.8mcg/Papaverine 17.44MG/Phentolamine 0.64MG). Picked up bottle on Oct. 6
Oct 7 - 12 units = put needle in then dropped it (from 11oclock to 7 o'clock angle), needle still stuck in penis, injected, ZERO erection
Oct 14 - 12 units = 4 hours firm erection - then 8 more hours before I went to emergency room - then 2 more hours before they drained what seemed to be a quart of blood (see pic at https://ibb.co/k6k1XfT) out of my dick = total 14 hours
Oct 19 - saw urologist (no real help), spoke further with pharmacy

No Trimix for two weeks while healing - masturbation wierdly yielded cumming about one minute prior to orgasm

Nov 4 - 10 units = 3:45 hour firm erection
Nov 5 - 5 units = 30min medium erection
Nov 11 - 10 units = 0 erection (only difference was going further back on shaft)

So, at this point, I don't know what to do.
Half of the time the TriMix isn't working at all.
Yes, I am keeping it in fridge. I suspect the failures have something to do with not hitting the cavernosa, but I followed all the directions.
It was 35 days old when it didn't work on Nov 11th. It seems unlikely it has lost its potency.
I have sex once a week, generally in a group setting.

Thoughts? Advice? Help?

I've done 70 injections. The first one injection worked well. The second injection got nothing.

I have since learned that most guys that miss simply guess where their corpora cavernosa are. As a result, miss injecting into the erectile chambers.
I have since learned to feel the corpora cavernosa and havent missed since.

That's certainly a weak starter mix.

I don't know what directions you're following but it sure seems like your injection technique is bad. You're lucky you didn't damage your dick, assuming you didn't.

Read as many threads in the "Injections" sub-forum as you can. There's lots of good advice in there. #1) Remember, if the needle is in the correct spot you should get essentially the same plunger resistance as if injecting into the air. If there is pain as soon as you start slowly depressing the plunger then you're needle placement is wrong. Injections work great for a lot of guys but you're sticking a needle into your body and injecting a drug. Use care and be responsible. Why would you wait 12 hours before seeking medical intervention?

Agree with sogwap and bldolnk. I have small corpora and the clock method doesn't work well for me, so I feel for a corpus with a thumbnail.

Ive done about 350 injections over 6 years.

First, as mentioned many times here, a plumped penis makes the cavernosa target bigger and better to "hit". Usually some foreplay or mild jelquing does just fine, but on occasion willie is stubborn and needs a mild VED pump to plump up. This is critical.

Second, you have to feel two points or resistance and punctures - the first to the outer tunica sheath just below the skin and the second point of resistance into the sheath of the cavernosa. If I don't get that second puncture it leads to a misfire and can lead to a painful swelling in the between tissue layer and is unproductive. The worst is hitting the lower spongosa and nerve/uethra bundle.

Some people draw the needle back to make sure there is blood (in the cavernosa) but I do not do that, but just feel for two distinct punctures.

From what most are saying, it seems as if I was/am mostly MISSING the cavernosa.

I tried to follow the directions in the videos that I watched. When discussed with the urologist (who didn't want to mess around with my dick too much as it was so bruised at that point), she pointed out with a model the exact same areas that the videos did (essentially 2 inches back from tip, 10 or 2 o'clock)

I don't know how to "feel" for the cavernosa - any tips or tricks or hints would be appreciated.
I have never felt the "two points of resistance" really; I've been relieved that it has gone in rather easily with little pain.
I *HAVE* noticed that depressing the plunger is not easy, I think it's because the dosage is so small that I keep pushing even when it's done

In answer to the "why wait 12 hours" question, a number of factors:
1 - I have a larger dick that curves down. The erection curves down as well, so it is not uncomfortable under clothes or etc. It was only about 3/4 hard at 4 hours.
2 - At 2am (the four-hour mark) nothing in the city was open that sold Sudafed. I tried Benadryl and ice for a while
3 - Because it wasn't hurting, and was about 3/4 hard, I thought it might just go down during sleep
4 - Even at 12 hours, it was not hurting that much. It was the 12-14 time that it went from non-painful to agony

Discussed the matter with my doctor today. She is looking into prescribing me a few shots of phenylephrine, as a prophylactic post-4 hours, just in case.

I've missed before and honestly I thought I was doing it exactly right. Thankfully, not a lot of pain but just no results. I'm almost always injecting into a soft penis and I don't have a pump so sometimes I'll just play with it a little before injecting. It doesn't get me hard but it does thicken up a little. One of my friends watches while I inject so if I'm with her, she'll suck me for a minute which makes me nice and plump and then she'll get undressed or masturbate in front of me as I inject and the thicker penis makes it easier. I have noticed that even when I keep the Trimix in the freezer it still loses a fair amount of potency as it nears the expiration date. At least it seems that way unless I'm just "thinking" it's less effective.

That cross-section is useful to show the relative positions of the two corpora cavernosum, but bear in mind that it shows an erect penis, so it looks like the clock method should work - hard to miss. My limp corpora are thin, however: 0.2in(5mm), like a thick spaghetti strand, too small a target for the simple clock method to be reliable.

To inject the right side, I bend my shaft a little to the left with my left hand, presenting a convex surface to the needle. This brings the right corpus closer to the surface and I can run my right thumbnail up and down the shaft feeling for it. The tunica (the tough covering of the corpus) has no pain nerves so the needle passes through painlessly, but oddly and fortunately, my corpora give me a touch sensation when I push against them with my thumbnail, and that's how I feel for a corpus cavernosum.

I'm right-handed, but I've practiced injecting with the left hand, so to inject the left side, I reverse the process, using my left hand for the syringe.

The corpora have fluid connections in the last 2/3 of the penis length, so I was told to squeeze the shaft 6-8 times immediately after injecting to get some Trimix into the opposite corpus. This helps to make a symmetric erection.

I've used 6-month old Trimix refrigerated at 40F(4C) with no change in effectiveness, so 35-day-old refrigerated Trimix probably hasn't lost anything.

Injecting is tricky but can be learned with practice. I've injected about 300 times and still get a miss every tenth time or so. (Phenylephrine can be taken orally if you don't have the injectable. "Wal-Phed PE" behind the counter at Walgreens.) AMPTWO: keep describing your progress.

I'm going to guess that you're missing, which I've done before as well. The danger is that if you miss, you might assume that you didn't miss and that the dosage was too weak. Then you increase the dosage...hit it....and you've injected too much. For me, it is easier to inject when I'm partially hard and that's not always easy to accomplish. I don't use a pump but one of my friends doesn't mind doing some oral to get me part of the way there before I inject so a little foreplay with her makes it easier for me to find the spot. My first year injecting was mostly good but I made some mistakes to learn by.
