So those of you who I have interacted with in the past may remember I used to get 2 hours with 3-4 units of T-101. Then it migrated to 5. A year and a half ago it moved to 8. This year has been a horrible year for sex due to some issues with health that my wife has had, but we brought the trimix with us to a post-christmas trip. I was discouraged to find that now 12 units gets me maybe 1.5 hours of time and the last 30 mins is a slow deflation.
So...how does this happen when I haven't been using it that much? I can't imagine I developed a sudden tolerance? Today, I put in 13 units on my right side and I got super hard for about an hour (which lasted foreplay through a few positions of intercourse), and then when switching positions for the finale -- not hard enough.
I find this a dramatic degradation in the efficacy of the meds...not sure what to think.

It will lose potency over time especially if not frozen.

As with so many things in life, mileage may vary. I've read in forums here about men who have had wonderful results with intracavernosal injections (Trimix, Quadmix. Alprostadil), as well as Viagra, Levitra, PT-141...but my experience with them all was very disappointing. That said, I always froze my injection medications and closely followed the prescribing physician's/manufacturers' instructions, but even with escalating and maximum doses, over 7 years or so, I only experienced a handful of satisfactory erections capable of penetrative sex. I am healthy man in my late fifties and have an implant now but it is only because I had absolutely no other options.. For me, it eventually came down to a penile prosthesis or near complete celibacy. I wish I had more supportive and encouraging news, but unfortunately there are only limited modalities for treating ED and this was the sad reality for me and many other men.
58yo, healthy, fit man with long-standing ED and disappointing results with conventional treatment modalities. Now implanted (07/2024) with Tactra, malleable implant.

I have been on injections for over 10 years and just last spring after injecting up to 30 units I had to move to Quad-Mix. Thank God I've had good results at a dose of about 10-12 units. There are so many varying factors with ED, everything from your state of mind, to most likely the worsening of your underlying condition that's causing the ED. For me it's circulation having had a Cardiac By-Pass operation some 13 years ago. What I can say what has given me good results along the way was the use of a cock ring. There are whole threads on this forum outlining the use of cock rings with product recommendations. That was a game changer for me, and have been using one for the past 5 years or so. Another recent change I started doing was really plumping up my penis just before injecting..helps to see the "sweet" spot much clearer, and you already have some blood filling the CA. Try it either manually or if you have a VED device.. Injections are a bit of an adventure with some experimentation. Have you tried using a cock ring? That will help keep the Trimix in your penis and slow down any veinous leak you may have...BTW..1.5 hours to maintain a good erection is excellent in my book...despite all the comments on FT about having sex for 2+ hours, I've never had that happen...nor do I want it..The GF gets tired of it, and frankly so do I...So Good luck to you, try not to panic that'll only add to the ED.
70 year old legally separated retired health-care professional. ED began in my early 50's. Viagra worked great for about 5 years, then had cardiac by-pass surgery and eventually moved to injections about 10 years ago.

Maybe your cardiovascular health has slipped a bit recently and you just don't realize it ?
If that's the case you may have to up your dose of trimix

Why would you be panicking?
Your 57, had an artificial heart valve.
Limited sex this past year because of your wife's health issues.
Trimix seems to be working and was "super hard" for about an hour.
Give it a chance. 13 units is still quite small.
You had sex for an hour plus. Sounds to me like a win.
I get it in your 20's you most likely used to do 3 rounds of 20-25 minutes and have multiples and would have done more if you lady could keep up. Or if you were like me, I had trouble with one round.
On a serious note, It's possible your mixture was weakened especially if you haven't used it much.
You may just be out sysn expecting, who knows what.

If it seems like Trimix is becoming less effective a couple different things could be going on: the Trimix might be getting old. The prostaglandins (one of the three components of Trimix) breakdown over time. That’s one of the reasons it needs to be refrigerated and why some advocate freezing it when it won’t be used.
It’s also possible that your ED is progressing. Whether it’s arterial insufficiency or venous leak (or a combination) it might be getting worse.
But the dose you are on is still quite small and the dose you took still got you hard for an hour. Most partners tap out after someone thrusting inside them for an hour.
So, enjoy the sex when you can. When it’s time to order a new vial of Trimix consider that the fresh batch might work better than an older one. Consider a slightly smaller dose for your first shot of the fresh vial.

I suppose that could be it -- my exercise took a nose dive over the past 4 months. Just weird to prepare mentally for a 2 hour erection and get a 1 hour instead. We do a LOT of foreplay, which eats up a good chunk of that hour so I was scaled to 2 hours of woody.
What's truly weird is that a month ago, with the same vile, I did about 10 units and was hard for almost 2.5 hours. Since that point, it's pretty much gone down right after I orgasm, like a normal erection would.
It's the consistency (or lack thereof) that concerns me. It reminds me of when my ejaculate volume dropped in my later 40's -- it was a light switch moment. I went from shooting ropes to suddenly producing very little, and while it concerned me greatly, no urologist found it concerning at all. "less mess" said one female urologist. :/

Doctors will blame it on your "progressing ED" because of liability issues. They will seldom admit that the trimix that they are prescribing is contributing to progressive ED.
It is well-known that Trimix causes fibrosis and scarring over time in most guys leading to worse erections. Not my words, Dr. Eid's.
I am in very good cardiovascular health but had psychogenic ED. In 4 years went from getting 4 hour rock hard erections on 4 units of the weakest trimix - to getting maybe 20 minutes of not even being very hard on 40 units of super quadmix. I haven't had a date in several years because of this and have no option left except implant, which I am too afraid to get.
You are lucky you can still get an hour. If you can get a decent response from alprostadil only("monomix") that might be your best bet to preserve function, since it is the other ingredients in trimix that are more damaging. Papaverine has high acidity and can cause fibrosis. But apart from this, each injection destroys some of the delicate erectile tissue in the corpora and there is no way around this really.
I think the guys on here who have been using injections for many years are mostly using alprostadil alone.

While I've been strictly an alprostadil (PGE1) user I don't think you're correct as to forum members. I have no compiled statistics but from comments I vaguely recall, most were tri-mix users with even some bi-mix users thrown in. I do think straight alprostadil is a little safer overall, although none of the mixes are risk free.
