Can a bad injection cause venous leak

I had one about a few months ago where it swelled up, and got black and blue. It seems that ever since, injections have not been as effective as they were before that. Just curious if you can actually cause a leak by an injection.

I doubt it, but really don't know.
I've hit veins and got the black and blue and localized swelling, but not overall swelling.
86 years
Inject testosterone weekly.
Implant on 1/22/19 by Dr Avila.
Scrotal, hor. incision just over 1"
18cm AMS 700 CX, 3.5cm RTE 100cc res
Gleason 6 prostate cancer. Monitoring it for now.
Update: On my last biopsies the cancer wasn't found.
