When they say see a doctor if you have an erection for over four hours, does that include it being a semi erection or is that only if it's a rock hard painful erection? My feeling is if it fades into a semi erection that's a good indication that it's not ischemic. A non ischemic (high flow priapism) is not the same medical emergency as a low flow priapism.
The reason I ask is that I think I've finally found a dosage of trimix that works for me. It gave me a hard erection for about an hour, but it didn't fade right after that. About two hours after the injection I still had a semi erection. I took two pseudoephedrine and now at 2.5 hours it's 'fluffy' or swollen, but not really erect. If I stimulate it will get erect again. I know I'm probably in the clear, but it just makes me wonder where the line is drawn.
If someone has venous leak, wouldn't that make an ischemic priapism almost impossible as the blood is always flowing out?

No worries...it's not even CLOSE! I have had erections on Trimix for as long as 6 hrs....which is actually the REAL threshold for Priapism (not 4 hrs). They wax and wane between semi hard and hard during that period. This for over a year with >>>ZERO ISSUES.
Priapism is VERY PAINFULL...Rock HARD with NO LET UP...COLD to the touch...and discolored (purple). Drug companies want to cover their ass, so they suggest you see Dr AFTER 4 hrs. This factors in an additional 2 hrs to get treatment (6 hrs). The GOAL is to resolve erection within an ADDITIONAL TWO hrs >>>>so that's EIGHT hours to resolution!!! I haven't heard of anyone on TRIMIX with an erection lasting longer than this (8 Hrs). Bottom line......the 4 hr advise to go to ER is BULL.

One thing I've been thinking is the people that get into trouble are the ones that are taking ED drugs when they don't need them. If you have venous leak or something similar, then wouldn't that prevent an ischemic priapism where the blood gets trapped in the penis? The 6 hours or whatever is for a ischemic priapism where the blood is trapped in the penis and is not getting circulated, high flow priapisms aren't nearly as damaging.

I don't know why you refer to High flow type...has no relevance here....only ISCHEMIC (trapped blood) which results in oxygen deprivation and necrosis. Necrosis is known to occur when ischemia exceeds 12 hrs duration.
Your dick will not fall off if your erection lasts 6 hrs!!! Most (if not ALL) those here on the forum who inject have erections that wax and wane over the total duration. THIS ISN'T PRIAPISM.
As far as venous leak>>>you won't get hard enough (with injection) because it defeats the mechanism responsible for a *usable* erection. ERGO : PRIAPISM NOT A CONCERN.

While in school for paramedics, we were taught 4 hours. When this came up on this forum, I looked up the definition in medical dictionaries, not Macleans Magazine. Not 1 of the medical dictionaries mentioned a time. Definition from the Mayo Clinic: Priapism is a prolonged erection of the penis. The persistent erection continues hours beyond or isn't caused by sexual stimulation. Priapism is usually painful. Although priapism is an uncommon condition overall, it occurs commonly in certain groups, such as people who have sickle cell anemia. Jul 8, 2016

Drug companies define Priapism as 6hrs or greater and PROLONGED ERECTION as 4hrs.
FOUR hours is used as a *trigger* to seek medical care to insure proper treatment at 6 hrs (allowing 2 hrs to actual treatment) with an additional 2 hrs to resolution after treatment initiated. That's 8 hrs from onset to termination to insure no damage.
If one decides to seek medical treatment after 4 hrs....go for it . I never have and never will.
Having said all that...keep in mind a PRIAPISM has NO LET UP....that means 6 hrs ROCK HARD with no periods of *SEMI HARD* (bendable). In addition>>>PAIN...COLD...DISCOLORATION.
