I’m on day 8 of post-op recovery. I’m at the point where I feel almost no pain/discomfort when lying down but when standing wow am I in pain. Mostly in my scrotum but also my penis. Reminds me a bit of my vasectomy pain but this may be worse. I know some of this is from positioning/pulling at the pump as per my surgeon’s instructions but yow does it hurt. Ice helps but only for a little while.
I’ve seen a recommendation for the Separatec underwear here and I may go for it but it’s expensive and jockstraps are too constricting for me. Does anyone have any other advice/tips for supporting my hurtin’ boyz? More endorsements for Separatec — and any sizing tips — would also help.

First of all, day 8? I was still laying horizontal without underwear on da 8. Taking it easier than necessary was a luxury I could afford and well worth it. It speeds recovery.
In any event, to answer your question, I tried all sorts of underwear (not being able to afford the fancy stuff mentioned numerous times on FrankTalk) and find that compression underwear does a fine job of keeping my genitals "organized" and might help with the pain you mention. It also keeps my implant-enhanced penis out of obvious sight.
All the below solutions presuppose wearing the penis pointed up and held against the belly. None would work if you wanted to hang down.
Jock Strap/Athletic supporter. Your problem may be one of size. Too loose lets your testicles move around more than might be comfortable. Too tight, of course, is not good for circulation.
Bike shorts Light compression and some degree of padding.
Women's underwear, if stretchy enough does a fair job of keeping a man under "wraps". and well controlled. (Note: Women's undergarments seem to be made of better material than men's briefs and are cheaper, to boot). Shortcoming of women's briefs/panties is that they often are not wide enough in the crotch to cover the (obviously unexpected) scrotum. Boy shorts have enough leg to provide coverage to my scrotum. However, if you don't mind the feeling of a thong, turning briefs/panties around backwards provides ample coverage for a man's balls.
Underwear designed as "control garments", "foundation garments" or "shapewear". (Think "Spanks" - and they also now make Spanks for men, too) will keep a penis held close very well. If compressing relieves your pain, that would be a viable solution.
But the best advice I can give you is to stay horizontal another week if you can.

Thanks for all the advice. Best of all was the perspective. I was under the impression that the acute pain would be over after 3-7 days. I have mostly been lying down (and sitting up a bit). But wow did I get a rude awakening when I tried sitting at my desk chair today! Couldn’t get comfortable and lots of pain. And I went for a short trip to the grocery (I live alone and was running out of stuff) and I barely made it to the register. Pain made me nauseous. Home delivery for me from now on.

I’m 3 weeks post implant and had little pain or swelling after the third day. Due to prior lower abdominal surgery, my surgeon made a second incision for the reservoir placement slightly above my belt line. Reason I’m mentioning this is it impacted my underwear choice.
I found the Separatec the most comfortable. They give your scrotum good support walking around and the softness does not irritate my penis head (partially inflated) and does not irritate the abdominal incision. I tried larger jock straps and found them very uncomfortable. I ordered 3 pair initially (to try) and liked them so much ordered 3 additional pair. Yes, they’re little expensive but was worth the price for me. These are the ones I have. Good luck.

You can also look for David Archy brand "2 pouch" briefs on Amazon too. They're made by the same company as Separatec and are a little cheaper, plus they come more to the package than Separatec briefs.
Make sure if you get David Archy that you specifically look for "2 pouch" briefs because they have others as well that aren't like Separatec.

It tried the separate pouch briefs and trunks they work pretty good, but didn't really care for them, I have worn all kinds of undies pre and post implant the ones. I like to wear the fashion jocks, not the gym type, or the old school jock pre implant so I've used these post implant along with my bikini briefs and the "tighty whitey" type briefs. The tighty whites had the advantage of being available almost every where, and cheap so if you don't like them toss them.
For the most part I now wear the bikini type men's briefs, and I'm for sure not an underwear model unless you've ever seen a pot-belly underwear model

This underwear has special features inside which support the penis upwards. I would suggest you try on a size smaller than the one you are used to. For me, size M feels still a bit loose. I will try the size S. Anyway, I can walk with more confidence as the penis will not fall to the side and look like I got a huge erection.

I got underwear here that I got from Amazon, all of them have my dick sticking out because I was just too sensitive to actually wear any jockey underwear.
If you have some old jockey underwear, that is not ratty and the elastic is worn. re-purpose them and just cut out holes for your dick to stick out.
If all else, just walk around nekkid in your house, have a robe nearby if you have to answer the door and make sure to close your drapes
