Well, after my recent posts i have being studying a lot the penile implants (i´m an advanced Medicine student, future Uro probably) I read almost all threads in the implant category and at yahoo penile implant group and other forums to read real people experience, and of course i read lots of Medical Papers. I want to contribute with this incredible forum my conclusions. Sorry if my english is not perfect, i´m from Argentina

Causes


* Long time without erections--> as many heard : Use it o lose it , The penis loose elasticity and slowly degenerate, so if u don´t have an erection for 5 years, yes, you will probably have lost some size.


* Cavernosal fibrosis or Tunica albuginea fibrosis: Corpus cavernosum is the place where the cillinders are put, the scar tissue that generates there make it really hard to the surgeon, is not easy to remove, sometimes impossible. The scar tissue is solid and non elastic so it will contribute to penile length loss. The fibrosis of tunica albuginea (tissue that limit expansion) make it also less elastic.
Causes of scar tissue formation: 1) Peyronie´s desease 2) Penile Trauma 3) Cronic Blood flow problems (Diabetics) 4) Intracavernosal injections (those who are inyecting please take this into acount, check frequently your medic to control the formation of scar tissue in your penis) 5) After Explantation of an infected device o whatever the cause, the place where the cillinder was fills with scar tissue, now the surgeons take this into account and make salvatation of infected devices cleaning carefully the area and implanting AT THE SAME MOMENT a new device.


* Prostate Cancer Surgery--> The portion of the urethra that goes throw the prosate is cut, and the remaining portions reattached causing loss of length


*Head Blood flow--> After the surgery the blood flow to the head is diminished, so it will not get engorged. Some people can recovere it during the healing and after some months


*Surgeon´s meassurement mistake--> Years ago many urologist used devices shorter than the dimensions of the c.cavernsosum because they thought it was safer in order to prevent erosion. The erosion was common with older devices, the semirigid ones, with the new one´s is a infrequent complication. Cause the loss of length is a common cause of disatisfaction now the surgeons uses the cillinders that best fit the c.cavernosum, and with the devices that only expand in girth only, they can be put supersized a bit.


*Patient unreal expectations---> The penile final length post op, should be the patient STRETCHED FLACID PENIS, that´s the elastic limit of your member. During your surgery your penis will be stretched to the max and then dilate the c.c and put the correct cillinders, it´s simple. To many the stretched flacid penis is equal to the natural erection length(c.c fills with blood and expands) , but for someones is not.


*The surgery itself---> Your penis is wounded and contracts a little, give it time to heal and stretch!


*Weight gain---> The is a subjective loss of length: If you have no erections for sometime and during that time you become more fat, the visible part of your penis will be shorter.

Now: Some solutions!

1) Now, the selected patients for a penile implant are given a daily plan of vacuum devices for 2 months to stretch your penis to the max! You can gain up to 2" this is no joke! It all depends in how much you lost through time and in your anatomy

2) The fibrosis is really difficult to treat, but it can be partially treated by corporoplasty

3) Penis enhancement surgery: Lots of medic do not recommend it cause penis loose stability, but for short penis it might be a good option

4) Weight loss: Diet, Exercise or Surgery

5) Use the implant daily as soon as u can post op to stretch your tissue to the max, i read lots of experiences that after 6 month more or less, people gain length. A more important length gain is noticed with the LGX implant because it expands in length. Now, some surgeons in revisions implant a new device longer than the one before because the tissue had expanded due to the previous implant!

6) The PDE5 inhibitors can help to engorge the penis head. Some people uses a constriction ring around the base of the head.

Well mmmm this is it, i think i have written almost all the causes. I hope this helps to understand and with the expectations. If you think that i wrote something wrong, or that some info is missing or incomplete, please comment it!

Thank you for description and medical outlook. Your explanations, coming from a person seeking to provide these solutions as a medical professional, gives me hope that all surgeons can understand the anguish/dilemma that the patient is striving to deal with.
