My AMS 700 cannot be pumped. A Uro with whom I consulted opines the device has failed, and a revision is needed.
Based on my personal feelings coupled to some of what I’ve read on this site, I am leaning towards a malleable device. I am 74 y/o and type 2 diabetic.
As a result of the first implant, I lost just under 1.5 inches. No sin was cut off and I am frustrated as to why. Since I was no porn star from the get-go, my dilemma is all-too-real.
I am hoping to see Dr. Eid in Manhattan. I believe he is one of the best. I am interested in opinions whether Eid can "find" my lost length and restore it.
Thanks for your input.

I don't understand why some lose few and others lose so much. Theoretically shouldn't it stay the same size? Was it the doctor's mistake?

I believe the reason some lost some size and some lost a lot of size is that as patients they all had varying degrees of erectile dysfunctions. I'm not smart enough to remember the medical words for all this, but ED weakens the tissues in your dick and they scar down more and more and more as you keep getting older and ED keeps getting worse and worse.
So, if one dude had a 7" dick in his 20s, but ED starts in his 40s and his erections start becoming weaker and weaker with worsening blood flow, then when he's 45 his dick is 6.5", then in his 50s it's 6", then by 55 it's down to 5.5"...I think you understand the concept.
Thus, if that guy chooses to get an implant when he was 55 then the implant goes on his 5.5" dick, making the result smaller as well. If he'd gotten an implant at 45 he'd have lost less size.
But now study after study has reported that this is a very solvable problem with the use of VED traction therapy to allow surgeons to install larger cylinders:
viewtopic.php?f=6&t=18561&p=169469#p169469
And here's a study reporting that yes, OP, you can recover your size. Buy a VED and start exercising it now.
viewtopic.php?f=6&t=20117&p=185488#p185488
Conclusion: if that dude above heals his dick with exercise, he could be 70 years old but get that 7" dick back with an implant, very much like it was in his 20s.

I feel that it’s a lot the surgeons skill level in in planting the proper size prosthesis. Some people who were sized aggressively report back to preop size shortly after they are activated . I on the other hand did VED daily and used shots for over a year and had maintained a 7” dick. Then after implant I was a hair shy of 6”. Gradually over the 5-1/2 months I’ve gained thru tons of perito exercises back up to about 6-1/2. Not one single time I pumped it up from the first until last nite did it ever hurt in the tips of the glans… I feel like I was undersized a little. Im very happy with what I have now… but feel like I could have gotten at least one or maybe two more cm when implanted. Some Drs are afraid of erosion and some think it’s ok to max you out. That’s my take on it. Maybe someday I’ll get a revision and get that xtra 1-2 cams and see if it’s positive bring it back after “X” years. Only time will tell

Re: Need revision - can I also recover lost length?
Sizing, we all want to know about size. As my doctor explained it to me. There is a hidden factor that they need to account for. Elasticity of your penile tissue. If you've lost elasticity then you'll get a slightly shorter implant. We all know that Peyronies & infection can cause scar tissue that then leads to shrinkage. Thus shorter implants. Well, loss of elasticity can lead to minor buckling of the cylinders if this isn't accounted for. Thus a tendency to use a shorter implant.

I always feel like correcting you but I'm worried I might end up wrong! Though I don't think I'll be wrong - the thing is, your signature says you got a 21cm implant. That equals 8.27". So you've got over 8 inches of implant in your dick, and the reason why it's technically longer than your total size is because the implant also goes underneath your pubic bone. My doctor explains it here:
If you stay at 6.5", that means almost 2 inches is stuck under your pubic bone, and I'm not an expert but I don't think it goes THAT far inside your body.
You're about 5 and a half months out. When I was there I measured right around 8.5" usually (well, I want to say "usually"; I think it's easy for us to ignore and forget about measuring smaller and blaming it on the time of day, temperature, our mood, etc. etc.). You measure at 6.5" now. In a week or two you'll be at 6.6". Another few weeks, 6.7". Just be patient and keep exercising it. Studies have reported the 6-12 months after implants have the highest gains. And also like you said, even if you do stay at 6.5" it's not like that's some epic tragedy; it'll work way better than before regardless, because your dick at least it gets and stays hard now. It's a clear win-win.

Yeah, it does.
I’ve looked in a bunch of human A&P texts and discussed it with my surgeon and regular urologist.
The typical range for the amount of penis hidden behind the pubic bone is between 25% (1/4) and 33% (1/3) of total length. There are exceptions in either direction. Those guys are not considered abnormal.
Rider’s reporting a 6.5” dick with 8.27” of implant. That gives him 1.77” behind the pubic bone. That’s 22%. A bit of a shallow crus, but not wildly outside the typical.
I’ve got 23 cm of implant. That’s 9.06 inches. I’m showing just over 6 5/8”. That makes my ratio 73/27…..very typical.
The most likely outcome for size…..is get a good BPEL (bone pressed erect length) before surgery using shots, pills, VED, whatever. Then knock off half a cm.
Obviously I’ve done no scientific controlled studies, but talking to a bunch of guys on here—-in the hand of a good surgeon, you’re going to be very close to original length, maybe lose 1/4” due to head rigidity.
Bottom line: in most guys, what’s going to end up visible is 2/3 fo 3/4 of the total amount implanted.
Age 68. Physically fit educated red neck in Texas. Very married. 23 cm (18+5) of LGX installed by Dr. Bryan Kansas 12/31/2019. I fought the ED and my wife & I won. I’m either full of shit or sound advice. You decide which.

Interesting about the pubic bone area. I believe you, just wasn't a thing I knew. Ty for the education, I'll learn more about it eventually.
The only thing I'd disagree with is that the most likely outcome for size is losing half a cm. What makes you say that?
Edit if you read this early: oh, well you kinda alraedy said what makes you think that, is that the majority on here expected about half a cm loss. Well, I think I just disagree with the majority here. The current medical research is miles ahead and consistent about size increases. And I experienced it like they explain. So I don't know what's stopping people, especially if they're using VEDs and at least exercising more than 15 minutes daily.

I totally understand distal and proximal measurements.. just seems like I’m doing a ton of stretching and pumping to get back to where I was. Where others I’ve read that were sized more aggressively we’re back to preop size almost immediately after activating. I worry that all the stretching and pumping it up for two 30 minutes periods a day now for over 5 months will cause premature wear on all the equipment

First I'd say I don't think I've ever heard of anyone getting back to pre-op size immediately. I'm not saying they don't exist, but I am saying I have never heard much from them. When I first got here on FrankTalk a year ago what I remember is people not even willing to talk about size. It seemed to be accepted that size loss was very possible, and maybe even likely, so naturally a bunch of men weren't too gung-ho talking about having smaller dicks. The overall benefits of having implants seemed to be the focus.
As for your concerns about pumping leading to premature wear, well first I was going to say how durable I believe the devices are, and this seems mostly believable that places like Coloplast already suggest daily cycling.
^the bottom is especially relevant if they’re already recommending 1-3 hours daily
Although there is a problem with believing what companies say, because I was going to cite how Coloplast also claims, “Durability: The Titan® IPP is built for outstanding fatigue resistance capable of 6.5 Million+ cycles (inflation and deflation) to material failure3, and provides 3x more Durablity.3”
That’s a pretty big claim! 6.5 Million+ cycles?
Had to find the study that came from. In their references it says they got the number from this study: Pritchard C., et al. Comparison of AMS 700 CX and Coloplast Titan Inflatable Penile Prosthesis Cylinders Subjected To In Vitro Cyclic Buckling
I search everywhere and I cannot find that study. I do find it mentioned here in an advertisement from Boston Scientific where they at least cite what journal it's in: https://www.bostonscientific.com/conten ... idence.pdf
Pritchard C, Daniels G. Comparison of AMS 700 CX and Coloplast Titan inflatable penile prosthesis cylinders subjected to in vitro cyclic buckling. J Sex Med. 2009;6(Suppl 1):4-41.
Oh, so they say it’s in Journal of Sexual Medicine 2009, volume 6. And since I still can’t find it in any searches I tried finding it in the journals themselves:
And I don’t think that study exists. Pretty sure they made it up.
So you probably shouldn’t believe what companies say about their durability, but I do believe that they’re really strong, medical grade devices that are not cheap. I asked them if I could buy a device itself because I assume if I could hold it by hand I’d realize they don’t break easy. They said they don’t, but your ask your doctor’s office and they should. Haven’t done that yet, but yeah, I get the feeling that for some men it’s not really the device that has them overly worried, it’s the fact that they’re in your dick. If you care enough about your size it's worth doing some work. And besides, what's the big deal anyway if it does weaken or happen to break? You can get it fixed and go without sex for a little while.
