One of the things involved is the engorgement of the glans (head), the more engorgement the better, some guys see no difference before and after implant surgery, others have little or no engorgement. The engorgement comes from a different blood source than the one which makes your penis hard. My engorgement is satisfactory and getting better. I asked my doctor about it, and I was told there was an ointment that will help the engorgement, but so far, I haven't needed that.
All aspects of sex with my wife are the same, we don't do anal. She says she can't feel the difference and we both have great orgasms. Oral sex seems to have a little less sensation than before, but like I said, it is improving. I am 3 months out from surgery and have been told it will continue to get better for about a year.
The normal progression of ED treatment is usually pills first until they fail or become unreliable, then injections until they fail or become unreliable, then the only option left is the implant. It is an
irreversible operation so you have to know what is or is not working for you now, if nothing is working for you then don't be afraid of getting an implant. The implant will improve your life if nothing else is working for you, but all the options should be tried until the implant is the only option that is left.
I have not regretted my decision for one second, I am very pleased with my implant and would not want to go back to having a limp dick.

Used Viagra & Cialis until lost vision in one eye due to AION, therefore can never use pills again, then tri-mix 1 1/2 years until unreliable. Implanted 9/20/22 at 77 years old by Dr. Yafi, UC Irvine. Married 55 years wife 76. 20cm Coloplast Titan.

I have had my implant for 3 years. I had a full functioning cock 9 months before the implant but a prostatectomy ended all that. My pre rp erection was a 10. On that scale I would rate the implant an 8. The sensation is all the same. 1000 times better than what I had just after the RP.

Well, it is partly true, your natural, beautifully functioning dick most likely wont come back and implant is a good solution for you to try.
But actually it is irrelevant to compare your implanted dick to severely dysfunctioned just before implanted dick. Because if you reached a point to consider an implant, most likely pills and/or injections are not working properly or you cant use them for some reason and you cant have sex properly. So anything; even a lifeless, cold, numb dick would be better because it will allow you to have sex.
So, comparision of pre-ED dick and implanted dick might be partly irrelevant, as even if it is %25,%50 or even %100 worse than you pre-ED dick you dont have really so many options, but it is also partly relevant to compare as you might set your expectations better and have a higher level of satisfaction after surgery

Here's my experience:
My implanted dick is warm to the touch. I have more life in it now than I've had for years. When I'm excited, I feel it swell around the implant. And I have powerful orgasms again after years of mediocre ones.
My ED began at 32. At 51, when I got my implant I didn't expect my body to do many things like it could at 32. So, I was hopeful, but realistic.
The satisfaction rate for implants of both the men and their partners is much higher than either pills, pumps, or injections. I am extremely happy with the results so far.
I hope this helps clarify my perspective.

Interestingly, I still get a 50%+ erection even with the implant, so there is still some blood flow going on. The corpus spongiosum is unaffected by the implant so the head and belly of the penis still engorge. I beg to differ with the cold, lifeless, dick assertion... at least in my case.
To answer your question: What is the worst thing you need to be prepared for? The worst thing is you need to pump it up to get a full firm erection. Of course I'd rather have full normal blood flow but I don't. Depending on your situation you may need to find ways to cover this if you don't want it to be obvious. I generally incorporate into foreplay while I'm going down on her. It's a win win. I needed to hear that. My implant date is Jan 4th, and well, frankly, I’m very anxious. I had a dream last night, very vivid, where somehow i’d learned that my venous leakage would rectify itself by cessation of smoking and a diet heavy in fresh squeezed vegetable juice. (both of which are true for me. I was an episodic smoker, smoke a few months, then stop for months, i’ve stopped for several months now. And i juice. Probably the reason my subconscious pulled this up in the dream). I woke up and realized it had been a dream. Which i don’t believe.
So i’m proceeding. Lately i’ve found myself landing on some of the less unpleasant accounts here on frank talk. And of course, my mind goes to these horrific what-ifs. (“am i just mutilating myself and setting my gate to a lifetime of suffering?”).
And then, i read about “90%+ patient satisfaction “ stats across multiple sources. I’ll read posts from guys who say “my only regret is that i didn’t do it sooner”. Man, i want to be one of those guys. I’m otherwise healthy. Regular in the gym. Lean and muscular at 63 (T-pellets) + relatively healthy lifestyle). Love sex still, but am affected by how my dick seems to be dying in advance of its owner, and not ready to retire from this pleasure.

From the Massachusetts Male Aging Study: The prevalence of ED in men is 52%. By age 40, approximately 40% of men report having ED. By age 70, nearly 70%of men report having ED. The prevalence of complete ED at age 40 is 5%, and by age 70 it is 15%.
From the National Health and Social Life Survey: Men between 50 and 59 are three times as likely to have ED as men between 18 and 29.
So, my ED started at 32 as the result of an injury. I had a fully functioning dick up until then.
It's possible that, even without the injury, by 51, when I got my implant, I would have been experiencing some degree of ED.
So, I'm going to continue to stick with my original position. The most relevant comparison, for me, is between my implanted dick and my dick on October 25, 2020.
