My doc said AMS would likely cover warranty for an implant that fails, which would be costs related specifically to the device. But the patient is still responsible for covering costs related to hospital and surgeon fees.

This makes the most sense. Many everyday items that require installation have warranties that match this. I see no reason to "read" an implants warranty differently. Yes, the implant is covered but the rest isn't. Of course if insurance is involved then that is a different picture.
