My drs nurse (female). Said that few men are very successful keeping their penis pointing up. I went through a few variations until I found a pretty good one. Even the nurse liked it. I bought a package of athletic supporters. More commonly called jock straps. I would position my penis straight up & then roll up two wash clothes into a cylinder. Placing it crossways in the jock strap to hold my penis up. This would be horizontal just above my scrotum. Then I used mens suspenders attached to my pants in the back. But I'd place the suspenders clip in the front capturing my pants & the waistband of the jockstrap. This kept the jockstrap pulled up tight thus pulling the wash cloths up tight against my penis.
I did wear loose pants & its not uncommon to see men wearing suspenders here. Especially older men. Experiment with how much padding needs to be in the jockstrap to keep your penis up. It takes a bit longer to pee & replace everything but for me it was very well worth it.

I currently have severe pain in the shaft and the tip, just below the glans. In the first two weeks, I was able to inflate a little every now and then as a test. I can't do that now, the pain is too severe. It's especially bad at night. In week 6 now, I can hardly put on trousers.
I recommend everyone to take 5-6 weeks off or, if possible, to work from home. I am not socially acceptable like this at the moment.
I'll try to have a warm bath tonight. Haven't done it yet. But my cut has healed perfectly so it should be fine.
I have my checkup and activation appt. with the doc on 9 October. But I can't imagine inflating currently.
I hope things will soften up soon, but is has not since surgery.
Currently life is very difficult.

Sorry to hear about your relatable pain as I just start my 6th week.
I took 4 weeks sick leave and yes going back to office work was not easy specially that I was bottomless post op at home. Did u try the double pouch underwear. They are a life saver and helped me a lot (there are 2 good brands at Amazon, David Archy & Separtec) in my case both were good and I almost tried all of the "double poches" in the market.
I had a hottible week 4 when my surgeon gave me the cycling session as I faced severe pain and even had seroma that made it difficult to navigate through the pump so asked the Dr to wait till week 6. I went back on week 5 and it was better but I was checking if I'm having an infection due to swollen sac and I was told that I'm OK..he tried to convince me to try cycling and I said no just deflate me and that's what he did (just a little) since I'm still unable to do it myself.
Reading all of the experiences here shows a light a the end of the tunnel and I'm being optimistic and only doing my best to stay away from infection and device malfunction.

Thanks for your reply. Yes I tried both of those double pouch underwear, I have both brands at home. However, I don’t see how they help or suppose to support me in my situation. I get the scrotum pouch and that part is comfortable.
But the penis-pouch doesn’t make sense at all. You can stick your penis in the hole inside and it will point your penis like 3 o’clock. You can even stick your penis through the opening at the front so the tip is „outside. But that doesn’t help me: When wearing pants, I need to „tuck“ my penis upwards to be able to wear pants. I cannot bend it downwards, it’s just „blocked“ and it hurts. Ticking upwards hurts too, but possible to some extent. So I need underwear that is very tight at the top to keep the penis up! Those pouches for the penis are just useless for me.
May I ask you how long it took until your seroma resolved? I think that’s exactly what I have around my pump. Just had a hot bath and it helped indeed a lot!

I guess I forgot to state that I'm not wearing suits nor tights pants so that's why they worked well for me.
My sroma started to get better on week 5 but not yet healed. Prior to that it was very painful and covering my pump specially the deflation button making the deflation process almost impossible and when my surgeon forced his way through I stopped my self from screaming out of of pain!
Good thing that I feel it's getting better on my 6th week and will look forward to experiment with cycling and understanding how to deflate since the more I try, I feel like I'm not getting there and still hard!

To give a little update. I am now 6 weeks and 3 days post-op.
Until last week I was still in extreme pain when I had to wear underwear and pants. That has improved a lot since pretty much the end of last week (6 weeks mark). It no longer feels like I have a penis filled with broken glass.
What did I do?:
- Warm full baths really help! Daily is best. I can completely massage away the seroma around my pump and also lightly inflate it.
- I bought some Diclofenac pills. These have helped enormously with the pain, much better than Ibuprofen, Paracetamol or Novalgin.
Even now I haven't taken a tablet today and the pain is almost gone.
Next week I have my official activation appointment with my Dr.

Hello dear FT members,
I know I haven't posted any updates for a long time. One reason for this is that I didn't want to write anything until there was really something worth reporting (yes, now there is a lot...). And it's also true that after the surgery and recovery, you simply spend less time on the forum and try to live your life.
To be honest, I was also a bit annoyed by the forum, as I received a lot of PMs, which I was happy to answer in detail. Often I didn't hear back for weeks/months or sometimes nothing at all, not even a "thank you". That's why I don't want to answer any more PMs. I'm sorry for all those who were serious in their PMs. But my time is simply too precious for that.
Now to the actual topic: The UPDATE!
As you know, I was operated here in Germany in mid-August 2023, Titan, peniscrotal. The operation went well, I had virtually no swelling, not on my scrotum or penis. I had virtually no pain for the first 2-3 weeks. I was also at home during this time and was able to recover. Most of the time I simply didn't wear any pants and underwear and spent most of the time on the couch or in bed. My wife supported me well. It was and still is unusual to have such a large penis all at once. But you get used to everything. Penis should be kept upwards.
It was and is weird to feel the plastic cylinders and tips in your penis, and of course the pump, which can be uncomfortable. It is what is.
From week three I had to go back to work, office job, suit, slim fit, lots of sitting, 8-10 hours/day. That's when the problems started. The fact that I had to wear pants again made everything very uncomfortable. I was in pain all day at work because I had to "squeeze" my penis into my pants. Standing, walking and sitting was very painful. Everyone could also clearly see that I had something in my pants, it has got better in the meantime, but the "buldge" remains. It was only when I got home in the evening that I was able to take my pants off and it was a relief. I would advise everyone to take 6 weeks off work if possible or to work from home. That would have saved me a lot of pain.
The second problem was that from week 3, when I went back to the office, a relatively large seroma formed around the pump within 1-2 days. This was probably caused by the tight clothes and the movement. The seroma is a kind of bulging, somehow self-contained bag of fluid around the pump, like a balloon. It was like having a golf ball in your scrotum and to operate the pump I had to somehow "massage" the seroma away, upwards. Apparently there was a connection between the seroma and the base? I don't know... in any case, I could massage it away and then activate the pump. However, the fluid flowed back into the seroma within seconds. The seroma only disappeared magically in the second half of December, after nothing had happened for months. In the meantime, my Dr. and I even wanted to plan another procedure to drain the seroma. But all of a sudden it was gone, which was a great relief.
Due to the pain and the seroma, sex was out of question for a long time. It was only after about 4 months that we took the plunge and it was fine.
The implant does what it is supposed to do. It works perfectly. I have no problems at all with the glans. Even when I'm not aroused, so the glans isn't engorged, but I'm still having sex, nothing kinks or anything. I thought that might be a problem at first, but it's not. I haven't lost any length overall and the penis is very hard.
My erection angle differs, depending on the situation. When I'm not aroused, it's between 2-3 o'clock (more towards 3 o'clock). When I'm aroused and fully pumped, it's around 2:10. What I don't like so much is that the penis no longer stands up so "tightly". It's difficult to describe. It's totally hard, that's not the problem. But when I push it "down" with my fingers in fully pumped status, I now have less resistance than before, when my erections were still possible without any problems. But of course, overall it's a 100% improvement on the previous ED state.
My wife and I are only slowly getting used to the new situation. It's strange that it's all happening again at once. You have to get used to the fact that the penis is no longer soft.
But you also need to know that you will always have a "big" penis, even when flaccid. And it is slightly harder than a natural penis. And yes, you can feel the tips of the implants when you press a little. But if you just touch the glans, then I don't think so.
I have virtually no pain now, not even when cycling, but that really took a while. I cycle 1x daily in the morning for 30 mins, as instructed by my Dr.
When I read in the forum that some people were able to get started straight away after 6 weeks... No way for me. Nothing worked for me then.
What I find interesting is that when I pump up the implant, I get about 25-30 pumps in. The last pumps are more like mini-pumps. Then I wait a few seconds and can pump in another 2-3 mini-pumps. That goes again and then it's over. At first I thought the cylinders were somehow losing fluid that I had to pump back in. But that's not the case. It gets harder and harder with every minipump, the minipumps are super important somehow.
All in all, I can recommend it. But yes, it's not perfect and it's a compromise. It works, but the first few months were no walk in the park. But I would do it again. Only the pump in the scrotum is sometimes a bit annoying and unfamiliar.
Important point: You can't see the pump when you're standing in front of me. But as soon as your partner touches the scrotum, she feels it IMMEDIATELY. You cannot hide the pump from a new partner, no way. Maybe if you tell her you are sensitive on your scrotum and she should avoid touching it.. You have to realise that.
The pump is there and is like a big marble, always there. Is that great? No, definitely not. But the whole thing is a compromise. It was worth it for me, I can now get on with my life and don't have to think about it all the time. By the way, I'm a passionate runner. I'm back on the track and can run 20 kilometres in one go without any problems. By the way, on of the most important advices I got was to STAY SLIM, slim belly, slim/no fat pad. Best for healing, and the outcome will be much better. So guys: Running or other kind of exercise is even more important than ever. It works for me. But I was always very sporty and I cannot imagine a life without running 3-5 times per week.
I now have sex maybe 1-2 times per week, not more often due to my low/non existent sex drive due to PFS. But I know and understand this will never improve again.

It was worth it for me, I can now get on with my life and don't have to think about it all the time. By the way, I'm a passionate runner. I'm back on the track and can run 20 kilometres in one go without any problems. By the way, on of the most important advices I got was to STAY SLIM, slim belly, slim/no fat pad. Best for healing, and the outcome will be much better. So guys: Running or other kind of exercise is even more important than ever. It works for me. But I was always very sporty and I cannot imagine a life without running 3-5 times per week.
I now have sex maybe 1-2 times per week, not more often due to my low/non existent sex drive due to PFS. But I know and understand this will never improve again.

Thank you wery much for your story. I'm 39y fit and going to have a ams700 near future. I'm wondering that if I ever gonna run after getting a implant but your story really encourages me.
Born1984 fit guy from Europe, back surgery and pain medicine caused a bad vl, using shots. All sex with injections at the moment. Considering to go ipp ams 700 surgery in the future...(public health care. I don't know the ams700 model or the surgeon)
