Trouble cycling.

Was cleared on Tuesday this past week to begin cycling and was given the instruction of how to do so. I have been easily able to feel the pump for the first 4 weeks and have done my best to keep it pulled down per the Doc's instructions but it seems to ride a bit higher now. I did my first solo pump Tuesday evening (about 10-11 pumps) and was able to hold it about 30 minutes. Deflated to a fairly comfortable flaccid and all seemed well. Next morning my scrotum was swollen on the left side where the pump sits and the pump seemed even higher....high enough that I can't easily feel the deflation bars and when I do think I'm there, there is a good amount of discontent from my left testicle. I am supposed to cycle a minimum of three times a week but am reluctant to pump up not knowing if I will be able to find the deflate bars. Ice and Motrin have been my solution thus far, but I am still too swollen as of this morning. Thoughts and suggestions are welcomed!
54 y.o. ED started 6-7 years ago with decreased response to meds. Trimix worked but pain, priapism, and need to plan everything made it a terrible solution. Implanted on 11/1/23 by Dr. Frederick Taylor, Columbus, OH. 26cm Titan XL (classic), infrapubic.

Hey Doc, first, congrats on your size. I also just got a 26 cm titan and loving it. I’ve only on day 5 but have started to cycle. Can you just pull down on the pump and it stays lower? Or does it just immediately come back up? Maybe if you tried positioning yourself differently when pulling down on the pump it would move the tubing a little to allow it to return to the bottom of your sack? Just a thought. Also wondering if the swelling is causing your sack to shrink, thus making the pump sit higher?

When I have tried pulling down on the pump it doesnt seem to change position at all. It sits only slightly lower than the testicles and behind the left one. I can't get ahold of the bulb without reaching pretty high up especially on the back.

Can you meet with your surgeon? If I were you I’d try to act fast on it before things settle. I’d feel more comfortable with the surgeon pulling on it and trying to relocate than me personally.

I have an appointment with him the second week of January. The NP I saw on my initial follow up did mention that occasionally they need to give a patient a little sedation and aggressively pull down on the pump to move it back into a better spot. I think part of the reason for the upward migration is the fact that my Doc had to extend some of the tubing. ( This is what he told my wife shortly after the procedure) My measurements were 15cm distal and 11 cm proximal, hence the use of the 26 cm device. I had no idea I had that much internal and I'm not sure where the starting point for the measurement is. My pre op fully engorged, bone pressed measurements where 7.75" L and 5.75" G. I am hopeful that with cycling I start to recover that and am eager to be able to cycle the way I need to.

Maybe try pulling it down yourself first, and with time it will drop. I hope that works!

The 26 makes sense then. My size before was 8.25x 5.5. From what I understand though implant size doesn’t always equate to dick size. You’re still well above average and I’m sure you will be that size again after cycling. Congrats man!

I am IP too and had some swelling issues that made deflation tough.
First be sure to pull on the pump 2-3 times a day till is settles in the lower scrotum.
That said, I found early that lying down on my back helped greatly in deflation. Youll easily feel the delation mechanism. Its tough to describe how I approach the delation. It feels like there is a bundle of sensitive nerves where the pump is and if I catch it wrong it feels like I am squeezing the testicle itself OW!
So I gently push the pump with my right hand and move the deflate area of the pump to the left, away from my testicle. I hope that helps.
I found it took 2 weeks for cycling to become easy

Trouble finding deflate circle on pump. Went to doc. He said the pump had moved 90 degrees from original placement. He didn't say anything about that 3 days ago at 2 wk postoperative visit. Pump is also next to left ball. OUCH!!!!
