What are Rear Tip Extenders (RTEs) and what do they do?
Mentioned all the time, but I've never known what they are. I have 21 cm implant with 3cm rear tip extenders. What do the 3cm extenders do? Google is confusing, figured someone here could explain it a lot better.

During the implant surgery, the surgeon, after making an opening, uses a device to measure the "space" inside the penis where the cylinders are to be inserted. If they measure 24cm of length, and they have 21cm length cylinders, they add 3cm's of spacers to fill the length. Thus,3cm of rear tip extender.
Somewhat simplistic...hope that answers your question.

as we know additions are not a good thing, how come they don't make cylinders of all sizes? so as not to have to add additions?, after all, a prosthesis costs so much because they threw money at it to be able to invent it and not because the prosthesis itself has an exorbitant cost, they are just silicone cylinders and other inexpensive stuff

I wonder this too. When talking to doctor I asked if implants could support my rather long penis and he told me, "we've never had a penis too large for these" or something to that effect.
First guess is it would cost a lot more to provide all lengths of cylinders, like 15cm cylinders, 16, 17, 18, 19, etc. Works better to make these at cutoff points and use extenders to fill the rest if need. Not sure, just guessing.

I believe that a single piece is much more performing, they should also do half a centimeter, I don't think an implant costs them over $ 200, doctors always tend to be super heroes, I have seen long implanted penises that barely rise

Well not exactly. First of all in the surgical guide for AMS 700 it is stated if the Crus cannot be dilated to a certain amount then RTE's (which are narrower) are used. It is a change in the way measurements are made and a shorter cylinder is used. I am not sure how this compares to the Coloplast as the AMS sizes are in 3cm increments and Coloplast 2cm increments. Second the softer cylinder LGX is made in a maximum length of 21cm and some doctors will not use this size as it has problems. So there is a limit at least for the LGX. And then where do you set your size limits to. Both minimum and maximum.
Now if the rep is required to attend the surgery with the sizes of implants it may be a problem to carry around a vary large number devices. Why AMS uses 3cm increments and Coloplast 2cm increments I do not know. So it is cost also in that the rep has at kit with cylinders and all the other components. So he can show up with a carry case or a truck with a big cart with the increases number of cylinder sizes. And AMS or Coloplast would have a lot more inventory of devices to carry on the books. Lets use the common sizes for an example in that AMS has 4. 12,15,18,and 21cm. If they carried cylinders in every cm they would have to carry 10 instead of 4. Now add that to CX and LGX and to the narrow cylinders and multiply by the number of reps around the world and the cost would be huge. A 150% increase in inventory to carry. If you went to 1/2 cm increments it would be even much greater. So I wouldn't look to see this happen. This is clear by Rigicon's selection of sizes seem a combination of AMS and Coloplast. And if you add infrapubic and penoscrotal variants along with preconnected and not you can see the crippling cost this would cause.

RTEs are used to facilitate putting the fill tube in the proper place. My cruse area is 3 inch deep. With out RTEs on me the tubing would travel 2 inches up into my cruse area along side the inflatable. With the 5 cm RTE the tubing enters the inflatable right next to my pubic bone.
