Hello all. I thought it would be beneficial to tell you all about my experience with my Titan penile implant and accompanying reservoir. I (gay male, 45 at the time) had the surgery at the end of February last year (2023), and I was really not prepared for how much pain I was going to be in for the next month. I have low pain tolerance, so obviously take that in to account as I go through all of this. I had severe Peyronie's with a 100° bend and plaque so was really only able to masturbate while only partially erect. From measurements I took when I was younger, I lost about 0.5 inches of length but pretty much retained my girth. I'm currently at 6 inches long and 6 inches girth.
Because of the severe pain I had while trying to sleep, and getting in and out of my regular bed I ended up renting a surgical bed for the month. Anything dealing with my lower abdomen was very painful, but more so when trying to get out of bed. I also became allergic to hydrocodone after a few days on it and was itching everywhere, but especially around the scars. I didn't realize that was what was going on at first because I thought it was just itching from my pubic hair growing back in. Ended up switching to just meloxicam and sometimes dealt with the itching from the hydrocodone if the pain was bad enough.
Per my doctor, I healed slightly faster than most patients (probably because of my age and overall health).
After about 2.5 weeks, my horniness won out and I stroked my head until I came. I was still too sore to stroke my shaft. It felt good but not totally orgasmic, more just a release and relief that everything was working. I also had to make sure I didn't thrust/move too much and did this while taking one of my sitz baths.
After completely healing (about 2 months or so), the implant scar around my penis at my original circumcision scar site had a lot of excess skin. This has since lessened but is definitely more than I originally had. Beyond that, no scarring is easily noticeable.
My husband and I started off slowly with oral sex and eventually moved to anal sex. I'm very thankful I had his support as I was definitely scared I was going to rip open the skin or otherwise injure myself. Here's where I will mention things I haven't really seen on this site. I have had sex with men and women and anal sex is definitely a little more difficult as far as penetration goes. With the implant not inflating the penis head, getting into the anus is trickier than it used to be when I was younger. The bottom (the one getting penetrated) has to be a little open to make this work. My husband and I have an open relationship and since my surgery I have had anal sex (as the top) with about 10 different guys. Newbies and inexperienced bottoms are very difficult to penetrate and I almost felt like I was going to rip off my penis head trying to get into one guy since it almost folded back on top of my penis.
The other issue I experience is with the bulb. My bulb fused to my scrotal wall while healing so it is permanently fixed in place. It also squeaks like a dog toy when I pump it which my husband and I joke about but obviously not something I like to have happen in front of other guys so I usually inflate right before I meet up with them. Most guys don't know that I have an implant and are really excited that I stay hard after I cum and can last until they're satiated.
I also drip cum for about 10 minutes, especially if I don't deflate. If I deflate right after cumming I will usually get another glob of cum and then I only drip for a few more minutes. This didn't happen prior to the surgery.
The last thing I'll mention is that I'm one of the rare guys whise device slightly leaks and therefore inflates my penis very gradually (completely deflated to about a quarter inflated overnight). I just deflate it completely in the morning, usually again on my lunch break and in the evening.
Even though I experienced a month of pain, I would (and will) have the surgery again. Being able to be the power top I was when I was younger has been amazing (and fun!). Feel free to ask me questions about the procedure, anal sex with the implant, and if you live in the Monterey/San Jose, CA area, I can certainly refer you to the surgeons who did the procedure.

Have you tried cialis or viagra for engorgement of the glans? I continue using cialis although now only use 5mg daily maybe a little more on sex days. It really helps with engagement of the shaft and glans for me. I ran out a few weeks ago and didn’t have any for about a week and man!!! What a difference!! Worth a try if you’re not using it now. I have a compounding pharmacy in town and even though it has increased lately it’s still very reasonable at $60 for 30/20mg dissolvable troches.
59 years old ED started mid 40s pills failed after 10 years. Injections works but diminishing results with pain. Implanted 5-22 Baylor,Scott,and White Dallas.Dr Michael Wierschem, infrapubic Coloplast 20cm and 1cm RTE. Going strong and loving it!

@Rider1400:
I haven't tried that but will ask my doctor about it. My glans still becomes engorged with blood flow but just isn't hard enough for anal penetration on tighter guys. Thank you for the recommendation!

If you're going to be speaking w/your surgeon about using ED drugs to get the glans engorgement you want, you should also ask him if it would be ok to use a VED (vacuum erection device) for this purpose as well or not.
FWIW, I'm a Bi top and, like you, the head of my glans does now get engorged w/inflation but, unlike you, I do not have any problem w/anal penetration because of it.
However, I recently started using a VED to regain some post-op size that I lost and doing so fully engorges my glans.
Another member here asked & got the ok to use a VED, as long (of course) he didn't "overpump" (ie., use excess pressure).
I didn't ask my surgeon for approval but I've experienced no problems using the VED daily in the AM & w/in 2-4 hours of when I expect to have sex.
If using a VED is something you'd like to explore further, PM me & I'll be happy to answer any questions that you may have about it.
Good luck!
PS: I'm in the SFBA too. My surgeon was Edward Karpman in Mt. View? Who was yours?

It’s great to hear from other gay men, and get their experiences. Thank you for sharing your update!

@sswinsfba
Thank you for the recommendation. I will certainly ask my doctor about using a vacuum pump as well. Dr. Karpman was my surgeon, assisted by Dr. Griggs in Salinas (who is my urologist here). Dr. Griggs is newer to the field and is assisting/working with Dr. Karpman until he is comfortable enough to perform the procedures himself here in Salinas.

I used mine for penetrative sex the first time a few days ago. It was awesome to not fumble with cock rings, etc. I had venous leak, so shots would make me hard only if standing, erection would go away in other positions. Also the pressure would make the erection go away. It was difficult working hopping up and down into the choreaography of a sexual act. Amazing.

Re: 1 year after penile implant - gay male perspective.
Thank you very much for your review. I'm 48 and probably going the implant route as I have Pyriones and had RP and severe ED after (almost a year with no natural response). Injections are painful and I haven't topped sucessfully since before my RP.
48yo gay married male - Prostatectomy done 7/2023. Severe ED due to nerve removal for possible cancer encroachment (none detected). Size before cancer / ED = 7.5" x 6.25"
